dc.description.abstract본 연구는 같은 연령대의 베트남여성과 베트남 혈연가족과의 비교를 통해 한국에 거주하는 베트남 여성 결혼 이민자의 건강 및 식이섭취의 변화를 알아보고 이에 영향을 미치는 사회경제적 요인을 알아보고자 하였다. 또한 이민여성과 베트남인의 식이섭취평가를 위한 FFQ를 개발하고 그 타당도를 평가하고자 하였다. 이를 위해 한국의 검진센터를 방문한 베트남 여성 결혼이민자 1471명과 베트남 하이퐁 지역의 검진센터를 방문한 여성결혼 이민자의 가족 또는 그 이웃 537명의 신체계측 및 혈액성분 조사를 실시하였다. 식이조사를 위해 이민여성은 1회 24시간 회상법을 이용하였고 베트남 현지 대상자들은 2회 24시간 회상법을 이용하였다. 베트남 대상자들의 특성을 조사하기 위해 베트남 대상자를 나이와 성에 따라 분류하였고, 이민여성과 베트남 대상자들의 건강 및 식이섭취를 비교하기 위해 20-30대 베트남여성 123명과 이민여성의 혈연가족 173명이 분석에 이용되었다. 또한 사회경제적 요인과 건강 및 식이섭취의 관계를 알아보기 위해 이민여성과 20-30대 베트남여성은 교육 및 수입수준에 따라 3그룹으로 분류되었다. 베트남 남성대상자의 평균나이는 37.7세였고 여성은 39.7세였다. 중성지방을 제외한 모든 신체계측 및 혈액성상은 남녀 모두 정상범위에 속했다. 또한 베트남 남성과 여성대상자의 모든 연령에서 에너지, 비타민A, 비타민B2는 베트남 권장섭취량을 충족시키지 못했다. 그러나 Vietnamese General Nutrition Survey와 비교했을 때, 대부분의 식품 및 영양소섭취는 베트남 남성과 여성대상자 모두에서 더 높았다. 현재 흡연과 음주를 하는 사람의 비율은 이민여성에서 9.8%, 14.8%였으나, 같은 연령대의 베트남여성에서는 흡연과 음주를 하는 사람이 없었다. 이민여성의 평균 BMI (20.7㎏/㎡)는 같은 연령대의 베트남 여성 (20.1㎏/㎡)과 그들의 자매 (20.2㎏/㎡)보다 높았다. 또한 이민여성은 같은 연령대의 베트남 여성에 비해 혈당, 총 콜레스테롤, LDL-콜레스테롤이 유의적으로 높았고 같은 연령대의 베트남 여성과 자매에 비해 중성지방과 HDL-콜레스테롤이 유의적으로 낮았다. 낮은 HDL-콜레스테롤의 유병률은 자매 (9.6%)보다 이민여성 (34.0%)에서 더 높았고 고중성지방혈증의 유병률은 이민여성 (10.6%)보다 다른 모든 혈연가족 (아빠: 56.3%, 엄마: 21.7%, 형제: 57.5%, 자매: 25.0%)에서 더 높았다. 평균 에너지 섭취량은 이민여성에서 1546.6kcal, 같은 연령대의 베트남 여성에서 1816.6kcal, 자매에서 1790.4kcal였다. 대부분의 영양소 섭취는 이민여성에서 더 낮았으나 지방과 식이섬유의 섭취는 같은 연령대의 베트남 여성, 그들의 자매보다 이민여성에서 유의적으로 더 높았다. 또한 이민여성은 같은 연령대의 베트남 여성, 그들의 자매보다 탄수화물에서 온 에너지 섭취량은 유의적으로 더 낮았고, 지방에서 온 에너지 섭취량은 유의적으로 더 높았다. 이민여성은 대부분의 식품 (두류, 과일, 난류, 우유 및 유제품, 유지류, 음료, 식물성 식품, 전체 식품)을 같은 연령대의 베트남여성과 그들의 자매보다 더 높게 섭취했다. 이민여성과 그들의 베트남 가족구성원의 영양소 섭취에 대한 상관계수는 거의 모든 영양소에서 통계적인 유의성이 없었다. 일반적으로 영양소 섭취는 이민여성과 가족구성원보다 이민여성의 자매와 나머지 가족구성원의 상관성이 더 높았다. 사회경제적 요인은 이민여성의 건강 및 식이섭취상태에 영향을 미쳤다. 더 높은 교육과 수입수준은 일반적으로 더 높은 지방섭취와 관련이 있었다. 그러나, 베트남 현지의 베트남 여성에서는 교육과 수입수준에 따른 지방섭취에 유의적인 차이가 없었다. 또한 수입이 가장 높은 군은 수입이 가장 낮은 군에 비해 탄수화물로부터 온 에너지 섭취는 더 낮았고 단백질과 지방으로부터 온 에너지 섭취는 더 높았다. 이민여성과 베트남 현지 대상자들의 향후 역학조사를 위해 FFQ가 개발되었다. 이민여성의 FFQ는 80개, 베트남대상자들의 FFQ는 66개의 식품 또는 음식으로 구성되었고 이 설문지의 빈도는 9점 척도를 이용하였으며 1회 기준분량은 적음, 보통, 많음의 세 가지로 분류되었다. 또한 이민여성의 FFQ의 타당도가 평가되었는데, 24HR과 FFQ의 보정하지 않은 상관계수는 0.32 (식이섬유)-0.61 (탄수화물), 1000kcal 당 상관계수는 0.31 (지방)-0.55 (칼슘), 에너지 보정 상관계수는 0.33 (지방, 칼륨, 비타민E)-0.52 (단백질)였다. 결론적으로, 이 연구를 통해 같은 연령대의 베트남 여성과 그들의 가족과 비교함으로써 베트남 이민여성의 건강 및 식이섭취의 변화를 알 수 있었고 이와 관련된 사회경제적 요인도 알 수 있었다. 또한 앞으로의 역학연구를 위해 이민여성과 베트남 대상자들을 위한 FFQ가 개발되었으며 이민여성을 위한 FFQ의 타당도가 평가되었다. 이민여성은 같은 연령대의 여성과 그들의 자매에 비해 중성지방과 HDL-콜레스테롤은 더 높았다. 또한 이민여성의 전체적인 영양섭취상태는 같은 연령대의 베트남여성과 그들의 자매보다 낮았지만 지방과 식이섬유의 섭취는 더 높았으며 사회경제적 요인은 지방섭취의 증가와 관련이 있었다. 앞으로 추적연구를 통해 이들의 이민 후 질병양상에 미치는 식이의 영향에 대한 연구와 베트남인을 위한 FFQ의 타당도 검증을 위한 연구가 더 필요할 것으로 사료된다.;Vietnamese female immigrants have been increasing rapidly in recent years. This study was conducted to examine changes in health and dietary status after immigration of Vietnamese female immigrants by comparing those of same age group and their family members in Vietnam and related socio-economic factors. Also, food frequency questionnaire (FFQ) was developed for female immigrants and Vietnamese in Vietnam and validity of FFQ for immigrants was evaluated. The subjects were 1471 Vietnamese female immigrants to Korea and 537 Vietnamese in Vietnam. General characteristics, health behaviors, anthropometric parameters, blood profiles, and dietary intake were assessed. 534 Vietnamese subjects excluding over 60 years of age were classified into subgroups according to genders and ages. Among, 534 Vietnamese subjects, 173 were second-degree family members of 47 female immigrants in Korea. Female immigrants and Vietnamese females aged 20 and 39 years were further classified into 3 groups according to their level of education and income levels. The mean age of Vietnamese subjects were 37.7 years for males and 39.7 years for females. All anthropometric and hematological variables except for triglycerides were in normal ranges. Vietnamese Male and female subjects did not meet Vietnamese Recommended Dietary Allowance (RDA) for energy, vitaminA, and vitaminB2 for all age groups. However, compared to Vietnamese General Nutrition Survey in 2000, Vietnamese subjects showed higher intakes of most nutrient and food. Proportions of current smokers and drinkers in the female immigrants were 9.8% and 14.8% respectively, whereas there were none of them in Vietnamese females in same age group (N=123). Mean BMI of female immigrants (20.7㎏/㎡) was higher than Vietnamese females in same age group (20.1㎏/㎡) and their sisters (N=54, 20.2㎏/㎡). Female immigrants had significantly higher blood concentrations of glucose, total cholesterol, and LDL-cholesterol than Vietnamese females in same age group and significantly lower levels of HDL-cholesterol and triglyceride than Vietnamese females in same age group and sisters. Prevalence of low HDL-cholesterol was higher in immigrants (34.0%) than sisters (9.6%) and prevalence of hypertriglyceridemia was lower in immigrants (10.6%) than rest of their family members (fathers: 56.3%, mothers: 21.7%, brothers: 57.5%, and sisters: 25.0%). Energy intake was 1,546.6kcal for female immigrants, 1,816.6kcal for Vietnamese females in same age group, and 1,790.4kcal for sisters. Although intakes of most nutrients were lower in female immigrants, fat and fiber intakes were found to be higher in female immigrants than Vietnamese females in same age group and sisters. Female immigrants also showed significantly higher percentages of energy from fat, and lower percentages of energy from carbohydrate than Vietnamese females in same age group and sisters. Also, intakes of most food groups (legume, fruit, egg, milk and milk products, oil and fat, beverage, plant food, and total food) were higher in female immigrants than Vietnamese females in same age group and sisters. The correlation coefficients for most nutrients between immigrants and their family members in Vietnam were not statistically significant. Generally, nutrient intake was more highly correlated between Vietnamese family members and sisters than between Vietnamese family members and female immigrants. Socio-economic factors influenced health and dietary status of Vietnamese female immigrants. Higher education and income levels are generally associated with greater fat intakes. However, no significant differences in fat intake were shown according to education and income levels in Vietnamese females in Vietnam. Also, the highest income group of female immigrants had higher percentage of energy from protein and fat and lower percentage of energy from carbohydrate. For further follow-up study, FFQ for female immigrants in Korea and Vietnamese subjects in Vietnam were developed. FFQ for female immigrants composed of 80 food items and FFQ for Vietnamese in Vietnam composed of 66 food items. The questionnaires used scale from 1-9 for reporting frequencies of consumption and portions of those were categorized into three sizes (small, medium, large). Also, the validity of FFQ for female immigrants to Korea was evaluated. The correlation coefficients between 24HR and FFQ in immigrants were 0.32 (fiber)-0.61 (carbohydrate) for unadjusted, 0.31 (fat)-0.55 (Ca) for per 1000kcal, and 0.33 (fat, K, and vitamin E)-0.52 (protein) for energy-adjusted. In conclusion, it was possible to examine changes in health and dietary status of female immigrants to Korea after immigration by comparison those of Vietnamese females in same age group and their family members and related socio-economic factors. Also, FFQ for female immigrants in Korea and Vietnamese subjects in Vietnam were developed and validity of FFQ for immigrants was evaluated. Female immigrants showed lower blood concentrations of triglycerides and HDL-cholesterol than Vietnamese females in same age group and their sisters. Also, Fat and fiber intakes were higher in female immigrants than Vietnamese females in same age group and their family members, although overall nutritional status was poorer. Socio-economic factors were associated with a greater fat intake in female immigrants. Future follow-up studies are needed to examine influence of diet on disease patterns after immigration and to evaluate the validity of FFQ for Vietnamese in Vietnam.-
