보험심사 전문 간호사의 직무표준 개발

Abstract

우리나라 건강보험의 진료비 지불제도는 행위별 수가제로 주로 운영되고 있어, 보험자는 의료비 관리를 위해 재정안정을 우선시 하지만 의료계는 전문성을 가장 중요시 여겨 진료비 삭감이란 문제로 두 기관 간 갈등이 발생하였다. 이러한 문제점을 해결하기 위해 정부는 의료서비스에 대한 진료비 심사와 평가를 담당 할 기관으로 심사평가원을 설립하였고, 의료기관은 임상경력과 의학 · 간호학적 지식을 가진 간호사가 진료비 심사 업무를 담당하게 됨으로서 보험심사 간호사가 생기게 되어 간호의 영역이 확장되었다. 한편, 다양한 의료분야에서 전문적 의료서비스의 요구가 증대함에 따라 각 간호 분야에서는 법적 제도권 내에서 전문 간호사가 배출되고 있으며, 보험심사 분야도 건강보험 급여의 적정한 청구와 평가를 수행할 전문 인력으로 전문 간호사 분야로 추진하고 있다. 그러나 현재 보험심사 전문 간호사 역할에 따른 직무표준이 없어, 보험심사 전문 간호사의 직무표준을 개발함으로써 진료비 심사 및 청구와 평가 등 관련 업무에 바람직한 보험심사 전문 간호사의 실무수준을 제시하여 실무에 활용하고, 보험심사 전문 간호사의 전문성을 확보하고자 연구를 시행하였다. 본 연구는 방법론적 연구로서, 보험심사 관련 업무 및 전문 간호사 업무에 대한 국내외 문헌고찰을 통하여 예비항목을 작성하였고, 자문 집단의 검토를 거쳐 보험심사 전문 간호사 직무표준(안)을 완성하였다. 보험심사 전문 간호사 직무표준(안)에 대해 현재 의료기관 보험심사 부서장으로 실무경력 15년 이상의 보험심사 간호사회 임원으로 구성된 전문가 집단에게 2차례에 걸친 내용 타당도 검증을 실시하였다. 마지막으로 전국 종합병원 보험심사 부서에서 근무하는 주임이상의 실무자 112명에게 직무표준(안)이 추후 보험심사 전문 간호사가 실무 현장에 적합한지 여부를 확인하는 실무적합성 조사를 통해 최종적인 보험심사 전문 간호사 직무표준을 확정하였다. 내용 타당도는 내용타당도 지수(CVI : Content Validity Index)를 이용하여 점수화하였고, 그 외 전문가 집단 및 실무적합성 참여자의 일반적 특성은 SPSS WIN 12.0을 이용한 기술통계 방법으로 분석하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 문헌고찰을 통해 표준 10개, 기준 50개, 지표 81개, 활동 302개의 보험심사 전문 간호사 직무표준(안) 예비항목은 작성하였으며, 자문 집단의 의견 수렴 과정을 거쳐 표준 12개, 기준 46개, 지표 92개, 활동 418개로 구성된 보험심사 전문 간호사 직무표준(안)을 완성하였다. 2. 보험심사 전문 간호사 직무표준(안)에 대해 전문가 집단 7명에게 2차례에 걸친 내용 타당도 검증을 실시하였으며, 그 결과에 따라 직무표준(안) 중 지표 3개, 활동 9개가 삭제되어 표준 12개, 지표 46개, 지표 89개, 활동 409개로 수정되었다. 3. 수정된 보험심사 전문 간호사 직무표준(안)을 현재 전국 종합병원 이상에서 근무하는 보험심사 부서의 주임이상 실무자에게 향후 보험심사 전문 간호사의 전문적 업무를 반영하는 직무표준으로 실무현장에서 적합한지에 대한 적합성을 확인하였으며, 실무적합성은 93.14%로 나타났다. 4. 이상의 과정을 통해 최종적으로 표준 12개, 기준 46개, 지표 89개, 활동 409개로 구성된 보험심사 전문 간호사 직무표준이 확정되었다. 본 연구를 통해 확정된 보험심사 전문 간호사 직무표준은 우리나라 전문 간호사의 간호표준에 준하여 개발된 직무 표준으로서, 우리나라 전국 종합병원에서 근무하는 보험심사 부서 주임이상 실무자의 실무적합성 조사에 근거하여 향후 보험심사 전문 간호사가 수행할 전문적 직무표준을 제시하였다. 개발된 보험심사 전문 간호사 직무표준을 근거로 진료비 심사 및 관련 업무를 통해 의료비용 관리와 적정성 평가 등 적정 진료 관련 업무를 가장 효율적으로 담당할 전문 간호사로서 역할을 수행할 수 있을 것으로 사료된다. 또한 본 연구의 결과는 향후 보험심사 전문 간호사를 제도화하는데 활용되기 바라며, 보험심사 전문 간호사 직무표준을 근거로 보험심사 전문 간호사가 수행한 직무의 성과를 측정할 수 있는 기초자료로 이용될 수 있을 것이다. 아울러 보험심사 전문 간호사의 역할 확대와 전문성 발전에 크게 기여할수 있기를 기대한다.;As the demands for professional nursing care service are increasing in various fields of nursing, advanced nurse practitioners are being produced and the field of health insurance review is trying to employ advanced nurse practitioners to demand and assess proper payment of health insurance. However, there is currently no job standards for advanced and health insurance review so the purpose of this study is to develop job standards in order to propose proper job level of the advanced nurse practitioners and apply the results to the work and enhance the expertism. This study is a methodical study. Pre-items were established by the reviewing the literatures in national and internations wide regarding health insurance review and advanced nurse practitioners and the job standards for advanced health insurance review nurses were developed after the examination by the advisory group. Also, the validity of the job standards were verified by the professional group for two times comprising executives of health insurance review nurses association with more than fifteen years of experience. Finally, 112 working level employees above the assistant manager position working in insurance review department of domestic general hospitals examined the relevance of the job standards, which determines whether the job standards were relevant to the actual business or not. The validity was examined by using the content validity index(CVI) and the general characteristics of the professional group and the participants of relevance examination were analyzed by descriptive statistics using SPSS WIN 12.0. The results of this study are as follow. 1. The pre-items of job standards for advanced health insurance review nurses comprising of 10 standards, 50 criteria, 81 indicators, 302 activities were established and the final job standards comprising of 12 standards, 46 criteria, 92 indicators, 418 activities were determined after the examination by the advisory group. 2. The validity of the job standards for advanced health insurance review nurses were examined for two times by the seven professionals and 3 indexes and 9 activities were deleted which modified the job standards to 12 standards, 46 criteria, 89 indicators and 409 activities. 3. The relevance of the modified job standards for advanced health insurance review nurses was examined by the working level employees above the assistant manager position working in the general hospitals and the result was 93.14% relevance. 4. By going through the process above, the job standards for advanced health insurance review nurses comprising of 12 standards, 46 criteria, 89 indicatosr and 409 activities were developed. The developed job standards for advanced health insurance review nurses were developed in accordance with the nursing standards of domestic advanced nurse practitioners. Professional job standards that will be executed by the advanced health insurance review nurses were proposed based on the relevance examination by the working level employees above assistant manager position working in general hospitals. By the development of the job standards, it is expected to prove that advanced health insurance review nurses are the most suitable professionals for dealing with medical affairs such as management and propriety evaluation of medical expenses while defining the roles of advanced health insurance review nurses. In addition, the results of this study are expected to be used as the strategy to be recognized as systemized advanced health insurance review nurses. The job standards developed in this study could be used as the basic materials to evaluate the performance performed by the advanced health insurance review nurses.
