퇴행성 슬관절염 여성 노인의 성생활과 삶의 질에 관한 연구

Abstract

퇴행성 슬관절염을 가진 여성노인의 성생활과 삶의 질에 대한 관계를 분석하여, 대상자에 대한 이해를 높이고 간호부분에서 효과적인 간호중재 개발에 필요한 기초자료를 마련하는 것이 이 연구의 목적이다. 본 연구의 결론은 다음과 같다. 대상자의 질병관련 특성 중 무릎통증 기간은 0-4년이 43.7%로 가장 많았고 다음이 5-9년으로 25.3%였다. 무릎통증점수는 0-20까지이며 평균 9.14점으로 보통정도의 통증으로 나타났고, 강직은 0-8까지이며 평균 3.35점으로 보통정도의 강직이었다. 현재건강상태는 ‘나쁘다’가 56.4%로 가장 많았고, ‘보통이다’가 43%였다. 대상자의 25.3%만이 성생활을 하고 있었으며, 성생활빈도는 ‘1년에 2-3회’가 40.0%로 가장 많았고, ‘6개월에 2-3회’가 31.5%, ‘1개월에 2-3회’가 20%였다. 성 만족감 변화는 이전보다 ‘만족감이 적다’가 55%였고, ‘비슷하다’가 42.5%였다. 성생활을 하지 않는 대상자는 ‘나이가 들어서 성욕구가 생기지 않는다’가 대부분이었고, 성욕구가 생겼을 때 해결하는 방법은 다른 일에 몰두하거나 자신의 의지, 종교적인 힘으로 자제하고 있었다. 대상자의 삶의 질은 평균 점수가 142.59점이었고, 문항 최고 점수가 5점일 때 평균점수는 3.24점으로 나타나서 삶의 질은 중간이상이었다. 대상자의 질병관련 특성 중 무릎통증(r=-.191, p=.016), 강직(r=-.234, p=.003), 현재의 건강상태(t=-3.085, p=.002)는 삶의 질과 유의한 차이가 있었고, 무릎통증과 강직은 삶의 질과 역 상관관계가 있는 것으로 나타났다. 대상자의 성생활 중 성생활 여부(r=5.95, p=.016), 성생활 만족도(r=7.94, p=.001)는 삶의 질과 유의한 차이가 있었다. 이상의 연구 결과를 통하여 퇴행성 슬관절염 여성 노인의 무릎통증과 강직, 건강상태, 성생활 여부, 성생활 만족감은 삶의 질과 유의한 관계가 있음을 알 수 있었다. 따라서 퇴행성 슬관절염 여성노인의 삶의 질 향상을 위하여 무릎통증과 강직을 완화시키기 위한 보존적인 요법에 관심을 가져야 하며, 성생활이 삶의 주요한 부분임을 인식하고 성생활에 대하여 긍정적인 자세를 갖고 노인의 성생활에 대한 여러 프로그램이 개발될 수 있도록 노력해야하겠다.;The purpose of this study was to investigate sexuality and quality of life of elderly women with degenerative knee arthritis. A convenience sample was selected from Orthopedic Surgery clinic at A Medical Center in Seoul. The participants were 158 women over 65 years old. The survey was performed using questionnaire from April 3th to 30th, 2008. The instrument included the KWOMAC-KNEE(Bae et al., 2001) to measure the degree of pain and stiffness. The instrument for measuring quality of life which was developed by Ro(1988) and modified by Kim(1994). The data were analyzed by using SPSS/WIN 14.0 programs with frequency, percentage, x2-test, t-test, Fisher's exact test, ANOVA, Scheffe post-Hoc test and Pearson's Correlation Coefficient. The results of this study are summarized as follows. The mean of knee pain was 9.14 (0-20) and stiffness 3.35 (0-8). Twenty-five percentage of participants have sexuality. The frequent of sexuality is 2~3 times in a year. The change of satisfaction of sexuality was less than before. The mean of quality of life was 142.59. That means quality of life is more than average. There was no relationship between sexuality and knee pain, stiffness, duration of pain and health status of elderly women with degenerative knee arthritis. But knee pain, stiffness, and health status were related with the quality of life. The keeping and satisfaction of sexuality were closely related with the quality of life. The quality of life according to degenerative knee arthritis was related with knee pain(r=-.191, p=.016), stiffness(r=-.234, p=.003), health status (t=-3.085,p=.002), keeping of sexuality(r=5.95,p=.016) and satisfaction of sexuality (r=7.94,p=.001). In conclusion, this study showed that sexuality and the conservative therapy for relaxing knee pain, stiffness are important factor to improve the quality of elderly women with degenerative knee arthritis.
