화상캠프 프로그램이 화상아동과 부모에게 미치는 효과 연구

Abstract

본 연구는 화상캠프 프로그램이 화상아동과 부모에게 미치는 효과에 대한 연구로, 화상아동의 심리사회적 문제를 고찰하고 역량강화이론을 바탕으로 프로그램을 구성 및 적용하여 임상적으로 효과성을 검증하고자 하였다. 궁극적으로는 화상아동에 위한 더 효과적인 개입 프로그램으로 발전되는데 기초자료로 제시하고자 한다. 본 연구에서는 서울시에 소재하고 있는 B병원 화상센터, H대학병원 화상센터에서 치료받은 경험이 있는 화상아동 25명과 그들의 부모 25명, 총 50명을 대상으로 화상캠프 프로그램을 실시하였다. 화상아동을 대상으로 자아존중감 척도(self-esteem Scale)와 한국판 화상특이건강 척도(Burn Specific Health Scale - Brief ; BSHS-B)를, 부모를 대상으로 부모의 건강대처목록 척도(coping health inventory for parents)를 통해 평가하였다. 자료분석 방법은 SPSS 12.0을 사용하였고 효과성 검증을 위해 연구 집단의 사전?사후 변화를 비교하는 대응표본의 차이검증(Paired Samples t-test)을 활용하였다. 본 연구의 주요결과는 다음과 같다. 화상캠프 프로그램은 화상아동과 부모를 대상으로 자아존중감 향상 프로그램, 질환적응능력 향상 프로그램, 부모 대처행동능력 향상 프로그램으로 구성하여 실시되었다. 프로그램의 효과성과 관련된 결과를 살펴보면, 첫째, 화상캠프 프로그램에 참여한 화상아동의 자아존중감과 질환적응능력은 프로그램에 참여하기 전보다 참여한 후에 점수변화가 증진되는 결과를 나타냈으나 유의도 .05수준에서 통계적으로 의미있게 나타나진 않았다. 이는 초등학교 저학년이 포함된 화상아동의 경우 프로그램 효과성을 검증하는 평가도구가 적합하지 않았던 것으로 사료되며, 또한 단기간에 변화를 살펴보는데 한계가 있었던 것으로 분석된다. 둘째, 화상캠프 프로그램에 참여한 화상아동 부모의 대처행동능력은 프로그램 참여하기 전보다 참여한 후에 통계적으로 유의미한 변화양상을 보였다. 이는 본 프로그램이 화상아동 부모의 대처행동능력향상에 효과가 있었음을 보여주는 결과이다. 따라서 본 화상캠프 프로그램은 화상아동의 자아존중감 향상과 질환적응능력 향상에 효과성을 입증하지 못하였으나 화상아동 부모의 대처행동능력 향상에는 효과적이었음을 유추할 수 있었다. 그리고 본 연구는 연구 집단인 화상아동의 집단 특성을 고려한 프로그램을 구성하고 적용했다는 것에 참 의의가 있을 것이며, 역량강화에 대한 개념을 화상아동을 대상으로 하여 캠프 프로그램과 연결시켜 봄으로써, 이들의 긍정적인 자아형성을 돕는 개입을 시도하였음에 의의가 있다고 사료된다. 이와 같은 연구 결과를 토대로 향후 더욱 효과적인 프로그램의 활용이 이루어지게 하고자 다음과 같은 제언을 하고자 한다. 첫째, 후속연구에서는 본 연구를 바탕으로 화상아동을 위한 프로그램을 구성하고 실천하는 과정에서 다양한 집단 프로그램의 개발이 요구되며, 캠프 프로그램과 병행하는 방법을 고려할 필요가 있다. 둘째, 향후 프로그램에서는 캠프 프로그램 효과성 검증을 위한 측정도구에 대한 보완이 요구된다. 셋째, 본 캠프 프로그램이 종료된 후에도 지속적인 변화를 도모하기 위해서는 사례관리가 필요할 것이다. 끝으로 본 연구 결과가 화상아동과 가족에 대한 고유한 프로그램이 개발되고 사회복지적 실천이 활성화 될 수 있는 방안 마련에 도움이 되길 바란다.;This study aims to find out the effect of burn camp program to the parents and the child burn survivors such as psychosocial problems, testing the effectiveness and making the program based on the empowerment theory. Ultimately, it seeks to be the basic resource of the intervention program for the child burn survivors. This study held the camp for the child burn survivors with 25 the child burn survivors and 25 of their parents who had been treated in B Hospital and H Hospital in Seoul. The child burn survivors were evaluated by Self-esteem Scale and Korean version of Burn Specific Health Scale-Brief; BSHS-B and the parents were evaluated by coping health inventory for parents scale. To analyze the data, SPSS 12.0 and Paired Samples T-test which helps compare the changes of before and after. The main findings of the test are as follows: Burn camp program was for the child burn survivors themselves and the parents consisting of Self-esteem Improvement Program, Adaptability Improvement Program in the management of burn disease and Parents’ Adaptation and Response Power Improvement Program. The results that measured the effectiveness of the program are as follows: First, the self- esteem and the adaptability in the management of burn disease seem to have increased before participating in the program but it was not statistically significant. This is because the testing devices were not appropriate for the age level of the child burn survivors since there were very young children involved as well. And the testing devices are not very suitable for measuring the changes in short period of time. Secondly, the parents who participated in the camp showed that their adaptation and response skills have increased significantly. This also shows that the burn camp program had more effect on the parents than the child burn survivors themselves. This program and the study could not prove to have effect on the self- esteem and the adaptation ability but is meaningful in the sense that it came up with specific programs for the child burn survivors and implemented them to help in building up their self-esteem. Based on this study, the following things are suggested to help create more effective programs in the future: First, there needs to be more creativity and more applicable camp related programs. Secondly, the future programs need to have more effective evaluation tool to ensure the effectiveness of the programs. Thirdly, to ensure the effective and lasting changes, the case managements need to be continued. Lastly, it is our hope that this study will be the ground for the further development of the implementation of social work programs for child burn survivors and their families.
