Abstract
본 연구는 호르몬 대체요법에 따른 중년여성의 건강상태와 생활만족도의 관계를 파악하여 중년여성의 건강관리를 위한 기초 자료를 제공하기 위하여 실시하였다. 2003년 3월 1일부터 4월 20일 까지 서울시와 경기도에 거주하는 호르몬 대체요법을 3개월 이상 실시하고 있는 중년여성 91명과 호르몬 대체요법을 실시하지 않는 여성 85명을 대상으로 하였다. 측정도구는 Ware(1977)의 건강상태 척도와 최성재(1986)가 개발한 생활 만족척도를 사용하여 설문조사 하였다. 자료 분석은 SPSS 10.0을 이용하여 대상자의 일반적 특성, 건강관련 특성, 호르몬 대체요법 특성은 빈도분석, 호르몬 대체요법의 실시여부에 따른 일반적 특성은 x^(2) , 일반적 특성에 따른 건강상태 및 생활만족도는 t-test와 F-test 및 DUNCAN의 사후검증을 이용하였으며, 호르몬 대체요법의 실시여부에 따른 건강상태와 생활만족도는 t-test 및 삼원변량분석, 건강상태와 생활만족도의 상관관계는 Pearson Correlation Coefficient를 이용하였다. 본 연구의 결과를 요약하면 다음과 같다. 대상자의 평균연령은 56.1세이고, 평균 폐경연령은 48.1세였으며, 기혼여성이 92%였다 호르몬 대체요법을 실시하지 않는 이유는 암에 대한 공포 54.1%, 들어본 적 없음 21.2%로 조사되었다. 호르몬 대체요법의 시작 동기로 중년여성의 63.7%가 의사의 권유에 의해서로 조사되었다. 대상자의 호르몬 대체요법 실시기간으로는 1년 이상-3년 미만이 34%, 5년 이상이 33%로 높게 나타났으며 60세 이상 여성의 경우 평균 투여기간이 4년 5개월로 조사되었다. 대상자의 일반적 특성에 따른 건강상태는 50세 미만인 경우, 전업주부보다 취업여성인 경우, 가정 월소득이 500만 원 이상인 경우에 자신의 건강상태를 가장 좋게 평가하고 있었고, 일반적 특성에 따른 생활만족도는 60세 이전 여성이 생활만족도가 높았으나 유의하지 않았으며 대졸이상의 여성인 경우, 취업여성의 경우, 가정 월소득이 500만 원 이상인 경우에 자신의 생활에 더욱 만족하는 것으로 나타났다. 호르몬 대체요법의 실시집단(M=55.1)이 비실시 집단(M=51.7)보다 건강 상태를 좋게 평가하였고 호르몬 대체요법의 실시집단(M=76.3)이 비실시 집단(M=67.8)보다 생활만족도가 높게 나타났다. 즉 호르몬 대체요법을 실시하는 중년여성이 호르몬 대체요법을 실시하지 않는 중년여성에 비해 건강상태를 더욱 좋게 지각하고 있었고 생활에 더 만족하고 있었다. 또한 중년여성은 자신의 건강상태를 좋게 지각할수록 생활에 더 만족하고 있음을 알 수 있었다. 즉 호르몬 대체요법의 실시는 중년여성이 건강한 생활을 영위하는데 도움을 주고 자신의 생활에 더욱 만족하게 하는 요인으로 작용하고 있었다. 따라서 의료진은 호르몬 대체요법을 실시하고자 하는 중년여성에게 호르몬 대체요법의 장·단점에 대한 정보를 충분히 제공하고 중년여성의 건강상태를 사전 평가하여 중년여성이 스스로 신중한 결정을 내리는데 도움이 되어야 할 것이다.;This study was performed in order to provide middle-aged women with the fundamental materials for health care by understanding the relation between their health condition and their life satisfaction degrees according to hormone replacement therapy (HRT). From March 1 to April 20, 2003, it had as its objects 91 women residing in the city of Seoul and Gyunggi-do to whom the therapy was performed over three months and 85 women to whom it was not performed. A survey by questionnaire was performed by using a s measuring instruments Ware's (1977) health condition measures and life satisfaction measures developed by Choi Seong-jae (1986). By using SPSS 10.0 for the analysis of data, frequency analysis was used for the general characteristics of the objects, their Characteristics related t o health, and the characteristics of HRT, x^(2) for the general characteristics according to whether to perform HRT, and t-test o r F-test and DUNCAN'S posttest for health condition and Rife satisfaction degree according to the general characteristics. t-test and three-way ANOVA were used for health condition and life satisfaction degree according to whether to perform HRT and Pearson Correlation Coefficient for the correlation between health condition and life satisfaction degree. The findings are summarized as follows: It was indicated that the average age of the objects was 56.1 and that their average menopausal age was 48.1, and married women accounted for 92%. The reason for having not performed HRT was the survey that the terror for cancer accounted for 54.1% and that those who have never heard of it accounted for 21.2%. The motive of starting HRT was the survey that 63.7% of middle-aged women were persuaded by doctors. The period of performing HRT to the objects was highly indicated in such a way that the one from one year to less than three years accounted for 34%, and the one over five years 33%. In the case of women over sixty, it was surveyed that the average medicating period was four years and five months. For the health condition of the objects, in the case of working women rather than full-time housewives in the case of those less than fifty, the case of those having household monthly income over five million won showed a high condition. For life satisfaction degree, women less than sixty had a high degree, but it was not significant. The case of those who studied further after graduating from colleges, the one of working women and the one of those having household monthly income over five million won indicated a high degree. The group in which HRT was performed (M=55.1) showed a higher health condition than the one in which it was not performed(M=51.7). The group in which HRT was performed (M=76.3) showed a higher life satisfaction degree than the one in which it was not performed(M=67.8). That is, middle-aged women to whom HRT was performed were perceiving their health condition better and were more satisfied with their lives, compared to those to whom it was not performed. It could be also known that middle-aged women were more satisfied with their lives a s they perceived their own health condition good. Thus, it was revealed that the performance of HRT had the effects of helping middle-aged women to lead healthy lives and having them be more satisfied with their own lives. That is, a medical team will have to provide ample information on the advantages and disadvantages of HRT, evaluate the effect that each individual of the objects prepares for risk, and help her make a prudent decision.
