만성신부전 환자 간호의 질평가 도구개발

Abstract

환자에게 제공되는 간호의 질을 향상시키기 위한 노력은 어렵고도 또한 가장 중요한 과제이다. 간호의 질을 보장하려는 계획은 우선 간호실무의 표준을 설정하는 것이 필수이며 간호의 표준으로 선정된 요소와 간호요소를 측정하는 부분적 항목의 선정에서 부터 각 단계의 작업이 전문적이고 체계적인 과정이므로 질적 향상을 위해 전문적인 도구개발을 해야 한다. 이에 본 연구는 만성 신부전 환자의 질적 간호를 제공하기 위해 질평가 도구 개발을 시행하게 되었다. 본 연구의 질평가는 과정적 평가로서 이는 환자에게 무엇을 해주고, 어떻게 하며 그것이 어떤 순서로 시행되는 가를 제시해주는 것으로 만성 신부전 환자를 평가하는데 가장 적당하다고 생각되어 과정평가 방법을 사용하였다. 본 연구는 도구의 개발과정과 도구의 타당도, 신뢰도 검증으로 나뉘어 진다. 도구의 개발 과정에는 촛점 집단, 전문가 소집단 및 전문 평가단등 세 종류의 전문가 집단이 참여하였다. 개발된 도구의 타당도를 위해서는 서울에 위치한 4개의 종합병원에서 근무한 간호사를 대상으로 하였고, 신뢰도 검증을 위한 대상은 3차 진료기관에 입원치료를 받고 퇴원한 3개월 이내의 환자 19명을 대상으로 하였다. 타당도, 신뢰도 검증을 위한 자료수집 기간은 1995년 9월 20일 부터 10월 18일 까지 였다. 본 연구의 개발과정은 다음과 같다. 1) 8명의 임상 간호사로 구성된 촛점집단을 통한 도구 문항의 예비목록의 작성 2) 4명의 전문가 소집단을 통한 예비목록의 수정보완 3) 23명의 전문가 평가단을 통한 도구의 내용 타당도를 측정 4) 개발된 도구의 신뢰도 검증 5) 타당도, 신뢰도 검증을 거친 표준, 기준 및 지표를 기초로 만성 신부전 환자 간호의 질평가 도구 확정 본 연구의 결과는 다음과 같다. 1. 개발된 만성 신부전 환자 간호의 질평가를 위한 도구의 문항 1) 개발된 만성 신부전 환자 간호의 질평가는 5개의 표준과 각 표준에 따른 28개의 기준, 각 기준에 따른 130개의 지표등으로 이루어졌다 2) 대상자의 만성 신부전 환자 간호의 질평가 점수는 평균 68.8점이었다. 2. 개발된 도구의 타당도, 신뢰도 검증 1) 만성신부전 환자의 간호질 평가 도구로 개발된 표준에 따른 기준, 지표에 대한 타당도 점수는 4점 척도를 기준으로 하여 점수화하여 표준에 따른 기준의 28개의 경우는 3.72, 기준에 따른 지표의 경우는 3.77점으로 비교적 타당도 점수가 높게 나타났다. 2) 개발된 만성 신부전 환자 간호질 평가 도구의 28개의 기준에 대한 2명의 평가자의 Pearson Correlation Cofficient 를 본 결과 r= .72, r= .75로 각각 나타났다. 또 두명의 평가자의 영향을 분석하는 단일국면 교차설계(singe - facet crossed design)을 이용하여 평가자간의 일반화 가능도 계수는 r= .96으로 나타났다. 3) 개발된 도구의 기준 28개의 내적 일관성 신뢰도는 문항분석 결과 alpha 계수는 .7259로 나타났다. 이상의 결과를 참고로 할때, 보다 다양한 방법의 타당도, 신뢰도 검증을 하여, 객관적으로 우수한 도구를 개발하고 보다 많은 환자 간호의 질향상을 위해 계속적인 간호의 질평가 도구가 개발되어야 할 것으로 사료된다.;Provision of better nursing care to patients is a difficult but important task. The first problem for nursing quality improvement is development of evaluation tool for the quality of nursing care. This study tries to develop a patient care tool for patients with chronic renal failure. This study as a procedural evaluation of patient care, tries to show what, how, at what order to provide care to patients with chronic renal failure. This study is divided into process of development of tool, its reliability and validity. Among process of development of tool is focal group, small expert group and expert evaluation group. To develop approprieteness of tool, nurses working is four major hospitals is Seoul were selected. To evaluate the credibility of sutbjects, 19 patient who were hospitalized and discharged within 3 months were selected. The period for collecting data for reliability and valiability evaluation was between Sept. 20 to Oct. 18, 1995. The development process of this study is as follows ; 1) Make preliminary list of the tool by focal group consisting of 8 clinical nurses. 2) Modify and add preliminary list by 4 expert nursing panel. 3) Calculate content validity of the tool by 23 nursing expert panel of judge. 4) Verity relability and validity of the tool. 5) Finalize an evaluation tool for the quality of nursing care in chronic renal failure patient. The result of this study were as follows ; 1. Development of evaluation tool for the quality of nursing care in chronic renal failure. 1) The evaluation of this study was developed 5 standards, 28 criteria, and 130 indicators 2) Nursing care evaluation scores for chronic renal failure patients were average 68.8. 2. Verity reliability and validity of the tool. 1) 5 standards were divided into 4 point scale and according to 28 creteria, indicators of standard were 3.72 and of criteria were 3.77 2) Inter - rater reliability (consentaneity score) of the tool by pearson correlation coefficient between rates were r: .72, r: .75 and inter reliabilities by single - facet crossed design were r: .96. 3) The alpha coeffecient relating to internal consistency was .7259 over 27 items of 28 criterias of developed tool. Through this study, I'm sure that the developed tool for the quality of patient care in chronic renal failure patient will show the way of more improvement of the quality of nursing care and effective nursing intervention.
