江南區 醫療施設 立地에 관한 硏究

Abstract

본 연구는 강남구의 의료시설의 분포를 살펴보고, 수요자 측면에서의 형평성과 공급자 측면에서의 효율성을 고려하여 적지를 선정하고 그것을 현재 입지한 의료시설과 비교하는 것을 목적으로 한다. 이를 위해 본 연구에서는 종합병원·병원·의원의 의료시설에 대해 입지-배분(Location - allocation) 모델을 사용하여 입지분석을 하였다. 입지-배분모형은 공간적으로 분산되어있는 이용자에게 서비스 시설을 어디에 입지시키는 것이 최적인가를 제시해 주는 모델이다. 본 연구에서는 입지-배분 모델중 GIS software 인 ARC/INFO가 제공하는 6가지 모델중 병원 입지분석에 적절한 4가지를 이용하였다. 그 결과로는 첫째, 수요자의 입장을 고려하여 총이동거리를 최소화하는 것을 목적으로 한 P-median 모델과 Minimize total powered distance모델에 의해 선정된 적지는 강남구의 전지역에 비교적 고르게 분산되어 있다. 대체로 모델에 의한 적지와 유사한 분포를 보이는 지역도 있으나 ,실제 의료시설의 분포는 일부지역에 편중되며 의료시설이 분포하고 있지 않는 지역도 있어서 형평성에 있어서 문제를 보인다. 둘째, 공급자의 입장을 고려하여 수요자를 최대로 커버할 수 있는 곳을 적지로 선정한 Attendance maximizing모델에 의해 선정된 적지는 강남구중 소득수준이 높은 역삼동과 논현동 인 일대에 집중하며, 실제로 이 지역에 분포한 병원의 수도 많다. 셋째, 수요자의 입장을 고려한 Maximal covering 모델이 선정한 적지는 1㎞내에 응급환 자가 발생했을 때 커버할 수 있는 적지를 선정한 곳으로 일부지역에서는 모델이 선정한 적지에 실제 병원이 입지하지 않고 있어서 응급의료서비스를 적절히 제공하고 있지 못하다고 보인다. 이처럼 입지-배분 모델이 선정한 적지와 실제 병원의 입지는 어느 정도 유사점을 보이고 있다. 새로운 병원을 입지시킨다거나 기존의 병원을 이동시킬 경우에는 형평성과 효율성을 높이기 위해서 위의 모델이 선정한 적지를 고려하여 새로운 입지를 결정하는 것이 좋을 것이다.;This study attempt to find optimal location of medical facilities in the aspect of efficiency and equity using location-allocation model. Since the study area, Kangnam-gu has been developed in 1970's along with the migration from the northern area of Han river, a lot of medical facilities established in 1980's-1990's. For finding the optimal location, population, the amount of property tax per person, road, landuse were used. The results are summarized as follows: First, the optimal location which is proposed by minimizing total distance model(P-median, Minimize total powered distance model) are distributed regularly in Kangnam-gu. In general the real location of medical facilities are located regularly, but in Yeoksam-dong, Nonhyun-dong the medical facilities are aggregated slightly. So, there happened any medical facilities is not found in some optimal location, and the problem of equity occurred. Second, the optimal location proposed by attendance maximizing model are centralized in high property tax area(like Yeoksam-dong, Nonhyun-dong). And there are many facilities in such area. So the actual location satisfy efficiency. Third, the optimal location proposed by maximal covering location model are distributed regularly. But the medical facilities are aggregated in some area so some area is disadvantage in emergency services. In conclusion, the real location is similar to the ideal location proposed by location-allocation model generally. But in some area no medical facilities are found, so if we consider location-allocation model for moving or establishing medical facilities, we can improve more efficient and equitable location of medical facilities.
