노인전문간호사의 치매환자 간호 표준 개발

Abstract

Owing to social changes such as women working and the nuclearization in modern Korean families, nursing for people with dementia in hospitals or facilities has added an extra burden. Nevertheless there is not a standard which takes a triangular position in the practice of dementia care, and previous research is deficient. The purpose of this study was to develop a standard for dementia care as a basis line for professional nursing, in order to improve the quality of nursing practice, and provide the framework for evaluation. This methodological study develops the dementia care standards based on identifying the nurse's roles, and tasks of caring for people with dementia through a literature review and survey using focus groups. The field survey was conducted on 195 nurses who were working at 36 dementia and geriatric hospitals in the nation, from October 9th to 28th 2006.

According to the delphi method, the data analysis was conducted using the Content Validity Index(CVI).

The study findings are summarized as follows: 1. The twenty seven tasks and 104 nursing activities for people with dementia were identified. 2. The final set of 18 standards on dementia care which included 4 structural standards: 'organization of nursing system', 'operating system', 'management of human resources', 'management of material resources', 13 procedural standards: 'advanced assessment', 'nursing diagnosis', 'nursing plan', 'advanced nursing implementation', 'evaluation', 'education', 'consultation', 'counseling and cooperation', 'development of specialty', 'utilizing resources', 'nursing quality assurance', and 1 standard on outcome: 'evaluation of nursing tasks in dementia care, were developed through one round of content validity verification. 3. The final set of 55 criteria on dementia care was confirmed through two rounds of content validity verification. 4. The final 171 indicators were confirmed through four rounds of content validity verification. These standards should provide the evaluation criteria of dementia care service as well as practice guideline for nurses in order to accomplish various roles and tasks in dementia and geriatric hospitals Research on evaluation and verification of the quality of dementia care and comparative studies between hospitals and facilities should further be conducted for understanding the actual state of dementia care and to establish the policy relation for dementia care.;급격한 노령인구의 증가는 노인성 질환의 유병률 증가를 가속화시켜 전문의료인력에 의한 노인 간호의 필요성이 대두되었으며 이에 2003년부터 노인분야의 전문간호사 교육제도가 시행되어 2006년 제1회 노인전문간호사를 배출하였다. 여성의 사회진출과 핵가족화와 같은 사회적 변화로 인해 노인부양기능이 약화됨에 따라 치매와 같은 만성질환의 시설보호가 가중되고 있는 현재, 치매 및 노인전문병원에서 치매환자를 위한 노인전문간호사의 전문적인 간호가 요구되나 국내 ․ 외에서 개발된 노인 간호 표준에는 치매환자 간호에 대한 내용이 구체화되어 있지 않아 실무표준이 마련되어 있지 않으며 관련 연구도 미비한 실정이다. 따라서 치매 및 노인전문병원에서 노인전문간호사에 의한 치매환자 간호의 질을 평가할 수 있는 틀을 제공하기 위해 치매환자 간호 표준을 개발하고자 본 연구를 실시하였다. 본 연구는 치매 및 노인전문병원에서의 치매환자를 위한 간호 업무를 파악하고 이를 토대로 치매환자 간호 표준을 개발하기 위한 방법론적 연구이다. 문헌고찰과 초점집단조사를 통해 치매환자 간호 업무를 확인한 후 전문가 합의를 통해 수정 ․ 보완한 내용으로 설문지를 개발하여 2006년 10월 9일부터 10월 28일까지 전국 36개 치매 및 노인전문병원의 간호사 195명을 대상으로 현장조사를 실시하였다. 현장 조사 결과를 토대로 치매환자 간호 표준(안)을 개발하였으며 전문가 패널을 대상으로 델파이 방법을 이용하여 내용 타당도 지수(CVI : Content Validity Index)로 타당도 검증을 실시하였다.

본 연구의 결과를 요약하면 다음과 같다. 1. 치매환자 간호 업무는 건강사정, 간호진단, 간호계획, 전문간호수행, 평가, 간호기록, 기록의 관리, 치매환자 교육, 간호인력 교육, 간호제공자 교육, 교육자료 개발, 연구활동, 실무적용, 연구지원, 치매환자 및 가족 상담, 자문, 협력 업무, 자원 및 정보활용, 변화촉진, 역할모델, 자원관리, 안전관리, 간호 질 관리, 사례관리, 옹호, 윤리규정 준수, 윤리적 의사결정 조력과 같은 27개의 업무와 그에 따른 104개 간호활동이 확인되었다. 2. 치매환자 간호 표준(안)은 1회의 내용 타당도 검증을 통해 구조적 측면 표준 4개, 과정적 측면 표준 13개, 결과적 측면 표준 1개 총 18개 표준이 개발되었다. 구조적 측면은 ‘간호조직 구성’, ‘운영체계’, ‘인적자원의 관리체계’, ‘물적자원의 관리체계’에 관한 표준이며 과정적 측면은 ‘상급 건강사정’, ‘간호진단’, ‘간호계획’, ‘전문간호수행’, ‘평가’, ‘교육’, ‘연구’, ‘상담’, ‘자문 및 협동관계’, ‘전문성 개발’, ‘자원활용’, ‘간호의 질 관리’, ‘윤리’에 관한 표준이고 결과적 측면은 ‘치매환자 간호 업무에 대한 평가’ 표준으로 구성되었다. 3. 치매환자 간호 기준(안)은 18개의 표준(안)에 대하여 55개의 기준(안)이 개발되었으며 전문가 패널에 의한 2회의 내용 타당도 검증과정을 통해 수정 ․ 보완 후 최종 55개의 기준이 확정되었다. 4. 치매환자 간호 지표(안)은 55개의 기준(안)에 대하여 175개의 지표(안)이 개발되었으며 4회의 내용 타당도 검증 과정을 통해 삭제 및 수정 ․ 보완 후 최종 171개의 지표가 확정되었다.

본 연구 결과 치매 및 노인전문병원에서 수행되는 치매환자 간호 중 일상생활 기능 유지 및 증진과 안전한 환경 조성을 위한 활동의 수행도가 가장 높았으며 타 건강관리팀을 위한 교육자료 개발과 전문직 표준 개발 활동의 수행도가 가장 낮은 것으로 나타나 앞으로 치매환자 간호 업무를 수행하는 노인전문간호사는 교육, 연구, 치매환자 간호의 전문성 개발에 중점을 두어 직무를 강화해야 할 것으로 사료된다. 본 연구를 통해 개발된 치매환자 간호 표준은 치매 및 노인전문병원에서 노인전문간호사가 치매환자 간호 업무를 수행함에 있어 간호의 질을 평가할 수 있는 근거를 제공할 것이며 구체적인 실무지침 개발에 활용될 수 있을 것이다. 앞으로 치매환자 간호 표준을 이용한 질 평가 검증 연구와 시설별 비교 검증 연구를 통해 치매환자 간호의 질적 향상을 도모해야 할 것으로 여겨진다.
