신장비뇨기계 임상전문 간호표준

Abstract

현재 전문 간호사의 자격은 전문적 간호서비스의 제공에 대한 필요로 종양, 임상 및 아동분야를 새로이 신설하게 되었고 임상전문 간호사의 세부 영역으로 심장-호흡기계, 소화기계(비만 포함), 신경-근골격계, 비뇨-생식기계, 내분비계(대사 포함)로 잠정합의를 이루었으나 계속 논의를 하고 있는 중이다. 전문 간호사의 제도적 확대를 위하여 우선적으로 우리나라의 실정에 맞는 전문 간호사의 역할을 정립하는 것은 매우 중요한 일일 뿐 아니라 더 나아가 전문 간호사의 질적인 간호서비스를 제공하기 위해 간호업무 표준화 연구가 선행되어야 한다고 본다. 본 연구의 목적은 신장비뇨기계 임상전문 간호사의 역할과 업무에 따른 간호활동을 규명하여 신장비뇨기계 임상전문 간호사의 간호표준을 개발함으로써 바람직한 신장비뇨기계 임상전문 간호의 수준을 규정하고 간호의 질을 평가할 수 있도록 할 뿐만 아니라 교과과정의 기초 자료로 활용하기 위함이다. 본 연구는 방법론적 연구로 연구의 목적을 달성하기 위해서 먼저 문헌고찰을 통해 우리나라 임상 현장에 적합한 활동으로 구성 된 예비 문항을 작성한 후 초점집단의 검토를 거처 신장비뇨기계 간호표준(안)을 개발하였다. 개발된 간호표준(안)은 현재 총 임상 경력 10년 이상이며 신장비뇨기계 분야에서 5년 이상 간호업무를 수행했으며 석사학위 이상의 학력을 소지한 10명으로 구성된 전문가집단으로부터 내용타당도 검증을 받았고 전문가 집단의 합의를 도출하는 과정에서 3차례에 걸쳐 내용타당도 조사를 실시하였다. 자료분석은 내용타당도 지수(CVI: Content Validity Index)를 이용하였고 그 외의 일반적 특성은 기술통계 방법을 이용해 분석하였다. 마지막으로 신장비뇨기계 분야에서 근무하고 있는 실무자에게 개발된 간호표준(안)이 추후 임상실무 현장에서 적합한지의 여부를 확인한 후 최종적으로 신장비뇨기계 임상전문 간호표준을 개발하였다. 본 연구결과를 요약하면 다음과 같다. 1. 신장비뇨기계 임상전문 간호표준은 대한간호협회(2004)가 제시한 전문 간호사의 핵심능력을 근거로 하여 전문가적 간호실무 제공 -자료수집, 간호진단, 간호계획, 수행 및 평가- 의 5개 표준과 교육, 상담, 연구, 변화촉진, 관리, 자문 및 협동의 7개의 표준, 총 12개로 하였다. 2. 12개의 표준을 근거로 하여 32개의 기준, 73개의 지표, 549개의 간호활동으로 구성된 신장비뇨기계 임상전문 간호표준(안)을 개발하였다. 3. 개발된 신장비뇨기계 임상전문 간호표준(안)의 3차례에 걸친 내용 타당도 검증을 통해 기준안은 32개 항목 중 1개를 통합하여 31개로, 지표는 73개 항목 중 13개를 통합하여 60개로, 간호활동은 549개 항목 중 131개를 통합, 삭제, 수정 과정을 거쳐 418개로 구성된 간호표준(안)으로 수정되었다. 4. 수정된 간호표준(안)으로 신장비뇨기계 관련 분야에서 근무하고 있는 대상자 50명에게 추후 임상 실무에서의 적합성 여부를 설문조사하였고 그 결과는 98%로 나타났다. 5. 최종적으로 개발된 신장비뇨기계 임상전문 간호표준은 12개의 표준과 이에 따른 31개의 기준, 60개의 지표, 418개의 간호활동으로 구성되었다. 본 연구를 통해 개발된 신장비뇨기계 임상전문 간호표준은 앞으로 활동할 신장비뇨기계 임상전문 간호사가 신장비뇨기계 환자를 간호하는 임상간호사의 업무지침서로 활용되고, 바람직한 신장비뇨기계 임상전문 간호의 수준을 정하여 간호의 질을 평가할 수 있을 뿐만 아니라 교과과정의 기초 자료로 활용할 수 있을 것으로 사료된다.;At present, new qualifications of nursing specialist for the field of oncology, clinic and pediatric have been introduced due to the demanding service needs. For instances, cardiovascular-respiratory system, digestive(obesity, etc.) system, nervous-musculoskeletal system, urogenital system, and endocrine (metabolism, etc.) system were selected tentatively as the subordinates to the clinical nurse specialist but still under the discussion. It is important to establish the function of the nursing specialist for the proliferation of the institution, and moreover, the study of standardization should be preceded for the quality of nursing. In this aspect, the purpose of this study is to optimize the nursing service of the clinical nurse specialists in the field of kidney-urinary system, by way of developing the nursing standards draft in order to establish desirable nursing standards, to evaluate the nursing level and to use them as the reference materials for the related curriculum as well. The study comprises a methodological research and the process includes the investigation of the previous studies, the preparation of the standards drafting for the kidney-urinary system with the examination of the selected experts group well educated and experienced in this field, to be effective for the actual clinical environment. This standards draft was developed through the validity inspection and consensus among the experts group, supported by the related surveys, and the analysis was carried out using CVI(Content Validity Index) and other descriptive statistics, which was confirmed by the working group in the field in terms of the adequacy for the practical works. The results of this study are summarized as follows. 1. The preliminary standards drafting for the clinic nursing specialist in the field of the kidney-urinary system includes 12 standards, in which 5 practical nursing practices of data collection, nursing diagnosis, nursing planning, practice and evaluation, 7 qualitative practices of education, counseling, research, leadership(transforming palpation, management), advising and collaboration are comprised, based on the major faculties of nursing specialist according to the proposed guidelines of Korean Nurses Association(KNA) in 2004. 2. The preliminary drafting composed of 32 criteria, 73 indicators, and 549 nursing activities. 3. The preliminary drafting was revised as the standards draft consisting of 31 criteria, 60 indicators, and 418 nursing activities by evaluating the validity for three times followed by combining, eliminating, and modifying the individual content. 4. A specific survey was performed to assess the adequacy of the standards draft for 50 professionals working in the field of kidney-urinary system, which showed 98 % acceptance. 5. The conclusive standards draft for the clinical nursing specialist in the field of the kidney-urinary system includes 12 standards, subordinating 31 criteria, 60 indicators, and 418 nursing activities. The standards proposal developed in this study can be utilized as the guidelines for the prospective clinical nurse specialist in the field of the kidney-urinary system and also can be used as the measure to evaluate the nursing level by establishing desirable nursing standards, as the reference materials for the related curriculum as well.
