신장이식 환자를 위한 교육용 CD-ROM 개발

Abstract

현대 의학의 발전으로 인하여 인간의 생명이 연장되고, 질병의 양상도 급성질환에서 완전히 치유되는 것이 아닌 만성 질환으로 바뀌어 가고 있다. 만성 질환 중 말기 신부전증은 신장의 장기간에 걸친 구조적, 기능적 이상의 발생으로 다시 정상으로 되돌릴 수 없는 상태로, 신장의 기능이 점차적으로 상실되어 불가역적인 상태가 되면 보존 요법으로는 더 이상의 치료가 불가능하고 투석이나 신장이식을 받아야만 생명을 연장시킬 수 있다. 신장이식은 1954년 미국 하버드 의대의 Murry 교수팀이 일란성 쌍생아를 대상으로 성공적인 이식을 시행한 이래, 새로운 면역억제제의 개발 및 환자 관리 방안의 발전으로 인해 전 세계적으로 이식 사례가 증가하였으며 말기 신부전증 환자를 위한 최선의 치료방법이나, 신장이식 수술 후 관리가 제대로 이루어지지 못하면 거부반응과 잦은 감염 그리고 고혈압 등의 합병증으로 심각한 사태를 초래할 수 있기 때문에 신장이식 환자들은 교육을 통한 신장이식 후 올바른 지식 습득과 이행이 필요하다. 본 연구는 신장이식을 받은 환자를 대상으로 신장이식 후 관리 방법을 Seels & Richey(1994)의 교수-설계 모형에 따라 신장이식 환자를 위한 교육용 CD-ROM을 개발하였다. 본 연구의 결과는 다음과 같다. 1. 신장이식 환자 교육 요구도 조사 결과 간호사는 감염 관리, 일상생활 관리, 거부 반응 관리, 면역억제제 복용, 건강관리, 합병증 순이었고, 신장이식 환자는 합병증 관리, 거부 반응 관리, 면역 억제제 복용, 감염 관리, 건강관리, 일상생활 관리순으로 높았다. 2. 신장이식 환자 교육 목표는 첫째, 신장이식 후 생길 수 있는 건강 문제에 대해서 안다. 둘째, 약의 효과, 복용 방법, 부작용에 대해서 안다. 셋째, 퇴원 후 자가 관리를 잘 할 수 있다라고 정하였고, 교육용 CD-ROM의 제목은 ‘소중한 신장 새로운 삶’이라고 정하였다. 3. 신장이식 환자 교육용 CD-ROM 내용은 교육 목표, 신장이식 후 건강 문제, 신장이식 후 건강관리 세 단원으로 나누었고, 각 단원별 항목은 신장이식 후 건강 문제 단원에서는 거부반응, 감염, 합병증에 관한 내용으로 구성하였고, 신장이식 후 건강관리 단원에서는 약물 복용, 매일 체크하고 기록할 사항, 정기적인 병원 방문, 외출은 이렇게, 치아 관리, 영양 관리, 운동, 여행, 사회 복귀, 성생활, 임신, 예방접종에 관한 내용으로 구성하였다. 4. Microsoft PowerPoint 2003을 이용하여 총 95장으로 구성된 신장이식 환자를 위한 교육용 CD-ROM을 개발하였다. 5. 개발된 신장이식 환자 교육용 CD-ROM으로 전문가 평가를 실시한 결과 전체 평점 4.00점에 대하여 평균 3.73±0.11점이었고, 내용 타당도 지수는 0.99로 높게 나타났다. 6. 수정․보완 후 최종 신장이식 환자를 위한 교육용 CD-ROM을 개발하였다. 본 연구의 결과 개발된 신장이식 환자를 위한 교육용 CD-ROM은 앞으로 간호현장에서 활용함으로써 신장이식 환자는 올바른 지식을 습득하고 올바른 치료지시 이행에 도움이 될 수 있을 것이다.;Owing to development of the modern medicines, the human life span has been extending, while acute diseases are being replaced with the chronic diseases not completely curable. Among them, the end stage renal diseases is an irrecoverable the result of long-term structural and functional abnormality; the kidneys loose their functions gradually to be irrecoverable, and then, the patient could no longer be treated with the conservative therapies, and thus, could extend his or her life only with dialysis or renal transplantation. Since 1954 when professor Murry's team of Harvard medical school succeed in transplanting a kidney between identical twin children, the renal transplantation has been used world-wide owing to development of new immunosuppressive agents and patients' management. It may well be the best treatment of the end stage renal diseases patients, but unless patients should be well managed after renal transplantation, they may suffer from rejection and frequent infections and such complications as high blood pressure, and therefore, the patients with their kidneys transplanted should be educated to be correctly informed of such side-effects to prevent them. The purpose of this study was to develop an educational CD-ROM for renal transplanted patients in reference to Seels & Richey's (1994) teaching-design model. The results of this study can be summarized as follows. 1. As a result of surveying nurses and renal transplanted patients for their needs for education, it was found that the renal tranaplanted unit nurses enforced to know about most was 'management of infection', followed by 'management of activities of daily living', 'management of rejection', 'medication of immunosuppressive agents', 'management of health' and 'complications' in their order, while the renal transplanted patients wanted to be informed of most was 'management of complications', followed by 'management of rejection', 'medication of immunosuppressive agents', 'management of infection', 'management of health' and 'management of activities of daily living' in their order. 2. The goals of the educational program for renal transplanted patients should lie in 1) knowledge about the possible health problems after rental transplantation, 2) information about effects of medicines, medications and side effects, and 3) self-care of health after discharge from hospital. The educational CD-ROM was titled 'Valuable Kidneys and New Life'. 3. The contents of the CD-ROM were divided into three units: educational goals, health problems after renal transplantation, and health management after renal transplantation. The second unit was redivided into rejection, infection and complications, and the last unit was recategorized into medication, everyday checkpoints, regular visit to hospital, going-out, dental management, nutritional management, exercise, travel, return to the society, sexual life, pregnancy, and inoculations. 4. The educational CD-ROM for renal transplanted patients consisted of a total 95 chapters edited using Microsoft Power point 2003. 5. As a result of referring the CD-ROM to expert group for evaluation, it was an average 0f 3.73(SD=0.11) on a 4-point scale, CVI(Content Validity Index) was 0.99. 6. The CD-ROM was completed through several modifications and complementations. The educational CD-ROM developed by this study for renal transplanted patients is believed to be useful to nurses as well as patients for their information and correct compliance.
