노인 당뇨병 환자의 무력감과 삶의 질에 관한 연구

Abstract

노인 당뇨병환자의 무력감과 삶의 질에 대해 파악하고, 대상자가 지각한 무력감 정도와 삶의 질이 어떠한 상관관계가 있는지 규명하고자 하였다. 따라서 당뇨병을 가진 노인들이 보다 만족스러운 삶을 살아갈 수 있도록 하기 위한, 간호중재를 마련하는데 있어서 기초 자료로 제공하고자 시도된 서술적 상관관계 조사연구이다. 연구대상자는 서울에 소재하고 있는 S 종합병원 내분비내과 외래를 방문하고, 60세 이후에 당뇨병으로 진단받은 남녀노인 100명을 대상으로 하였다. 자료수집기간은 2006년 10월1일부터 11월 15일까지 총45일 간 이었으며, 자료수집 방법은 설문지를 이용한 면담 방식 이었다. 연구도구는 무력감 정도를 측정하기 위하여 정승은(1998)이 개발한 ‘노인의 무력감 측정도구’를 사용하였고, 삶의 질 정도 측정에는 노유자(1988)가 개발하고 김종임(1994)이 수정, 보완한 ‘삶의 질 측정도구’를 사용하였다. 자료 분석은 SPSS WIN 12.0을 이용하여 실수, 백분, 평균, 표준편차, t-test, one-way ANOVA, Dunnett T3 검정법, Pearson's Correlation Coefficient의 통계방법으로 분석 하였다. 본연구의 결과는 다음과 같다. 1. 노인 당뇨병환자의 무력감 정도는 평균 72.43점, 평균평점 2.69점이었고, 삶의 질 정도는 평균 130.84점, 평균 평점 2.97점이었다. 일반적 특성에 따른 무력감은 연령, 교육정도, 결혼상태, 동거인, 한달용돈에 따라서 유의한 차이가 있었고, 삶의 질은 교육정도, 결혼상태, 동거인, 한달용돈에서 유의한 차이가 있는 것으로 나타났다. 2. 질병 관련 특성에 따른 무력감은 간병인, 입원경험, 혈당조절방법, 당뇨병 합병증 유무에 통계적으로 유의한 차이가 있었고, 삶의 질 정도는 간병인, 혈당조절방법, 당뇨병 합병증 유무에 통계적으로 유의한 차이가 있는 것으로 나타났다. 3. 대상자의 무력감과 삶의 질 간의 관계에서 무력감과 삶의 질은 역상관관계로, 무력감이 높을수록 삶의 질이 낮은 것으로 나타났으며, 무력감과 삶의 질의 하부영역별 비교에서는 정서상태, 경제생활, 자아존중감, 신체상태 및 기능, 이웃관계, 가족관계 모두에서 통계적으로 유의한 역상관관계를 보였다. 따라서 무력감 감소를 위한 간호중재의 개발로 노인 당뇨병환자의 삶의 질을 증진시켜야 할 것이다.;The purpose of this study was to investigate the relationship between helplessness and quality of life and in older adults with diabetes. The subjects were a hundred male and female older adults sixty years old or more who suffered from diabetes now in one general hospital located in Seoul. The data were collected from Oct. 1 to Nov. 15 2006. Research instruments included an instrument for measuring the level of helplessness which was developed by Jung(1998) and an instrument for measuring quality of life which was developed by No(1988) and which was corrected and complemented by Kim(1994). The data were analyzed using SPSS WIN 12.0. The scores for the level of helplessness and quality of life were calculated by minimum, maximum, mean, and standard deviation. The level of helplessness and quality of life according to general and diseased characteristics were analyzed by t-test and ANOVA. Dunnett T3 test was used as a post-hoc test. Pearson's Correlation Coefficients were used for the correlation between two variables. The results from this study are summarized as follows. 1. The mean score of helplessness was 72.43 and the mean score was 130.84 for the quality of life. 2. The level of helplessness according to general characteristics made a significant difference in age(F=19.774, p=.000), educational level (F=20.723, p=.000), marital status(t=-4.962, p=.000), inmate(F=11.109, p=.000), and monthly allowance(F=22.838, p=.000). Quality of life made a significant difference in educational level (F=15.899, p=.000), marital status(t=5.198, p=.000), inmate(F=11.322, p=.000), and monthly allowance(F=27.662, p=.000). 3. The level of helplessness according to diseased characteristics made a significant difference in nurse(F=8.043, p=.000), hospitalization experienced(F=11.381, p=.042), a method controling blood sugar(F=4.754, p=.003) and whether a complication arose or not(t=5.637, p=.000). Quality of life according to diseased characteristics made a significant difference in nurse(F=6.796, p=.000), a method controling blood sugar(F=2.724, p=.045), and whether a complication arose or not(t=-5.578, p=.000). 4. There was significant negative relationship between helplessness and quality of life(r=-.823, p=.000). In conclusion, these result suggest that helplessness may be important factor that improve quality of life in older adults with diabetes. Therefore, older adults with diabetes should be provided nursing intervention that can decrease the helplessness.
