혈액투석환자 간호업무 표준개발

Abstract

최근 과학의 발달과 생활수준의 향상으로 인간의 평균 수명이 연장되고 있으며 식생활의 변화, 고도의 산업화에 따른 공해 문제와 정신적 긴장으로 인해 질병의 양상도 급성질환보다는 만성질환으로 그 형태가 변화하고 있다. 이에 만성신부전도 만성질환의 일종으로서 복막투석이나 혈액투석을 통하여 지속적으로 병원과 연관하여 의료서비스를 받고 있다. 이에 따라 혈액투석 환자들은 의료서비스의 질의 향상이 투석환자 자신들의 건강관리에 직접 영향을 미치므로 이에 대한 관심이 증대되고 있다(양영옥, 1995). 이와 같이 혈액투석 환자간호의 질적 수준은 매우 중요함에도 불구하고, 혈액투석 환자의 간호 질에 관한 연구는 매우 미흡한 실정이다. 이에 본 연구는 혈액투석 환자에게 질적인 간호를 제공하고 질 평가의 근거를 마련하며 혈액투석 간호사의 임상실무수행능력 향상을 도모하기 위한 혈액투석환자 간호업무 표준 개발에 관한 것이다. 본 연구의 진행은 미국간호업무표준과 미국신장간호업무표준 및 한국간호업무표준과 선행연구 등을 문헌고찰 한 뒤 예비문항을 작성하였다. 작성된 예비문항은 초점집단의 검토를 통해 혈액투석환자 간호업무 표준(안)을 개발하여, 그 타당도를 검증하기 위하여 전문가 I, II집단과 100명의 실무자로 부터의 3회의 검증을 실시하였다. 또한 실무적합성을 확인하기 위해 업무 수행률을 확인 하였다. 본 연구의 결과는 다음과 같다. 1. 대한간호협회가 제시한 간호표준개발 기본원리를 바탕으로 작성된 예비도구 문항에 대해, 초점집단의 토의 결과 12개의 표준, 32개의 기준, 91개의 지표, 750개의 업무 표준(안)이 개발되었다. 2. 1차 내용 타당도 검증 결과는 전문가 집단 I의 80%이상 합의율을 보였으나, 반복되는 내용과 항목을 삭제, 수정, 보완 과정을 거친 후, 12개의 표준과 32개의 기준 및 71개의 지표, 총 419개의 간호활동으로 정리하였다. 전문가 집단 II 로부터 2차 내용 타당도 검증한 결과, 전체 CVI는 0.98이었다. 전문가 합의율이 80%미만인 항목은 표준 3. 계획에서 기도흡인의 위험성 간호의 간호활동 항목 중에 '음식물 섭취 후 30분간 침대머리를 올려둔다'를 삭제하고, 지적된 21개의 간호활동을 추가하여 총 12개의 표준과 32개의 기준, 71개의 지표 및 간호활동 440개로 구성하였다. 실무자를 대상으로 3차 내용타당도를 검증한 결과, CVI는 0.988이었으며, 실무자 합의율이 80%미만인 항목은 없었다. 3. 개발된 혈액투석환자 간호업무 표준(안)에 대한 실무적합성을 확인하기 위해 임상에서의 수행여부를 조사한 결과 전체 업무 수행율은 88.5% 이었다. 4. 최종적으로 개발된 혈액투석환자 간호업무 표준은 자료 수집, 간호 진단, 계획, 수행, 평가의 업무표준과 윤리, 업무수행 평가, 교육, 연구, 협동, 자원 활용, 간호의 질의 전문직 성과표준으로, 총 12개의 표준, 32개의 기준, 71개의 지표, 440개의 간호활동으로 확인되었다 . 본 연구를 통해 개발된 혈액투석환자 간호업무 표준은 향후 혈액투석 간호실무의 표준이 되며, 이를 제공하는 간호사들의 질 평가를 위한 제도적 장치로써 사용될 수 있을 것이다. 또한 혈액투석 실무지침서 작성과 간호사의 업무수행 능력을 향상시키기 위한 도구로도 널리 활용될 수 있을 것으로 사료된다.;This study is a methodological research to develop standards for Hemodialysis nursing practice and to offer Hemodialysis patient qualitative nursing and it is thing about the development of practice standards for Hemodialysis nursing to ready basis of quality evaluation and improves practice performance ability of a Hemodialysis nurse. Progress of this study is to investigate Nursing standards from American Nurses Association(ANA), American Nephrology Nurses Association(ANNA) and Korean Nurses Association(KNA). After reviewing preceding research, made reserve content of standards for Hemodialysis nursing practice. Through examination of made reserve content by focus group meetings, standards for Hemodialysis nursing practice were developed and were verified validity of them by professional group I, II and 100 staffs in actual clinical environment. Also, confirmed their performance rate by working-level nursing 100 staffs in Hemodialysis unit. The results of this study are as follows. 1. About reserve content that is made with Nursing standards development that Korean Nurses Association(KNA) presents, result of discussion with focus group meetings composed standards of 12 standards, 32 criteria, 91 indicators and 750 nursing activities. 2. The first content validity verification result was shown the agreement rate more than 80% of professional group I, but repeated content and items. After pass through delete, correction, repletion process it was arranged 12 standards, 32 criteria, 71 indicators and 419 nursing activities. According to result that verify the second content validity from professional group II , whole CVI(Content Validity Index) was 0.98. Item that is less than agreement rate 80% 'head-up position in bed for 30 minutes after ingestion' in Standard 3 'Plan' was deleted and adds indicated 21 nursing activities and composed 12 standards, 32 criteria, 71 indicators and 440 nursing activities. According to result that verify the third content validity from 100 staffs in actual clinical environment, CVI was 0.988. and there was no item that is less than their agreement rate 80%. 3. The whole performance rate was 88.5% as result that investigate achievement availability in actual clinical environment. 4. The final version of the standards is developed 12 standards, and their subsets of 32 criteria, 71 indicators, and 440 nursing activities. The 12 standards were set based on two perspective; standards of nursing practice and those of professionalism. The former includes data collection, nursing diagnosis, nursing plan, nursing performance, evaluation; the latter includes ethics, performance evaluation, education, research, cooperation, resource utilization and quality of nursing. Standards for Hemodialysis nursing practice developed through this study gets into standards for Hemodialysis nursing practice hereafter, and may be used by systematic device for nurse's quality evaluation. Also, is considered that can be utilized widely by a tool to improve practice performance ability of Hemodialysis nursing practice guide.
