환아와 어머니의 회복력에 관한 연구

Abstract

암으로 진단 받는 아동은 해마다 증가 추세에 있고, 이는 아동의 생애 전반에 걸쳐 위협적인 결과를 초래할 수 있으며 지속적으로 관리를 해야 하는 만성 질환적 특성을 지닌다. 따라서 암환아는 정상적인 성장발달에 문제를 가져올 수 있고, 그의 가족 또한 신체적, 경제적, 심리적인 부담감을 경험하게 되며 가족 구성원은 가족 기능의 새로운 역할에 적응해야 한다. 따라서 본 연구는 환아와 어머니의 회복력을 측정하고, 이들의 상관관계를 파악하여 회복력에 영향을 주는 요인을 파악하고 나아가 회복력을 증진시키기 위한 간호중재의 기초를 제공하고자 시도되었다. 자료 수집은 2005년 4월 20일부터 5월 30일까지 서울 A 종합 병원의 소아암 병동에 입원하거나 외래로 치료 중인 11세-18세의 암환아와 그들의 어머니 각 각 60명을 대상으로 하였다. 연구 도구는 김동희(2002)의 만성질환아 회복력 측정도구, 채명옥(2005)이 수정 · 보완한 가족 기능 회복력 도구를 이용하였다. 수집된 자료는 SPSS 프로그램(Version 12.0)을 이용하여 빈도와 백분율, 표준편차, Chronbach's alpha 계수 및 t-test, ANOVA, pearson's correlation coefficient를 산출하였다. 본 연구의 결과는 다음과 같다. 1. 암환아의 회복력은 평균 98.32점(평균평점 3.090± .30)로 개인내적 특성은 (3.208± .38), 대인관계 특성(3.171± .37), 대처양상 특성(2.878± .39)였다. 어머니의 회복력은 평균 64.95(평균평점 3.08± .25)로 정서적 지지체계가 (3.300± .47), 가족 성숙(3.288± .39), 가족의 힘(3.128± .38), 재정적 추진력(3.067± .56), 외부자원 활용 능력(2.658± .56)였다. 2. 암환아와 어머니의 일반적 특성에 따른 회복력은 환아가 종교가 있을 때 (t=7.095, p=.010), 어머니가 취업상태(t=4.986, p=.032)일 때 유의한 차이가 있었다. 3. 환아의 회복력과 어머니의 회복력은 유의한 상관관계가 있었다(r=.058, p=.000). 회복력의 세부영역별 관계는 암환아의 대처양상 특성과 개인내적 특성은 가족의 외부자원 활용능력(r=.786, p=.002), (r=.287, p=.026) 과 유의한 상관관계가 있었고, 암환아의 대인관계 특성과 개인내적 특성은 가족의 재정적 추진력과 유의한 상관관계가 있었다(r=.608, p=.000), (r=.357, p=.005). 이상에서와 같이 암환아의 대처양상과 어머니의 외부자원 활용능력이 낮은 것으로 확인되었다. 암환아의 일반적 특성 중 종교가 있을 때, 어머니의 경우 취업 여부에 따라 회복력이 높음을 알 수 있었고, 암환아의 회복력과 어머니의 회복력은 밀접한 상관관계가 있었다. 따라서 암환아의 대처양상을 증진시키고 암환아 어머니의 외부자원 활용능력을 증진시키는 간호 중재로 회복력을 높여서 만성적인 질병과정 중에 있는 암환아와 가족이 힘든 과정을 잘 이겨낼 수 있도록 해야 한다. 임상에서 암환아를 간호하는데 있어 환아 뿐만 아니라 어머니, 가족을 포함한 간호를 통해 이들의 회복력을 함께 증진시켜야 한다. 또한 암환아가 종교 등의 신념체계를 통해 어려운 과정을 이겨낼 수 있도록 돕고, 가족이 외부자원 활용능력을 적극적으로 활용할 수 있도록 암환아의 진단, 치료, 간호과정과 관련하여 눈높이에 맞는 적절한 정보와 설명이 이루어져야 할 것이다. 본 연구의 결과를 바탕으로 추후에는 환아의 어머니 뿐만 아니라 아버지를 포함하여 전 가족을 대상으로 한 회복력 측정이 필요할 것으로 사료된다. 또한 암환아와 가족의 회복력을 증진시킬 수 있는 간호중재에 관한 후속 연구가 필요하다.;The purpose of this study is to evaluate resilience of children with cancer and their mothers. This study aims to identify the main factors behind the resilience of these patients and their mothers. To measure the resilience of cancer patients, Kim's(2002) resilience scale for chronic illness was utilized. The Korean Family Functioning Scale by Lee et al in 2002 and revised by Chae in 2005 was used to calculate the resilience of mothers. Respondents for this study consist of 60 pediatric cancer patients and their mothers. Data was collected from April 20th, 2005 through May 30th, 2005 at A Medical Center in Seoul, Korea. Data was analyzed by the SPSS (win 12.0 version) program with descriptive statistics, Chronbach's alpha coefficients, the t-test, ANOVA, and the Pearson's correlation coefficients. The majors findings of this study are as follows: 1. The resilience mean of pediatric cancer patients is 98.32(3.09±.03) and of mothers is 64.95(3.08±.25). 2. There appears to be a high resilience in pediatric cancer patients who hold religious beliefs(t=7.905, p=.010). Similarly, there is a high resilience for mothers who are employed(t=4.986, p=.032). 3. There is a significant positive correlation between the resilience of cancer patients and their mothers (r=.508, p=.000). As the study indicates that there is a definite relationship between the resilience of pediatric cancer patients and their mothers, it is vital to improve the condition of both the patient and his mother to augment the healing process. Furthermore, it is important to support the beliefs and religious activities of children and to ensure that mothers find effective ways in dealing with stress. Based on the results of this research project, a few changes are recommended for future studies. The follow-up studies of not only fathers but also the entire family members also be needed and a focus on characterizing the effective modes of nursing intervention.
