복막투석환자 간호업무 표준 개발

Abstract

본 연구는 복막투석 환자를 위한 질 평가의 근거를 마련하고 복막투석 간호사의 임상실무 수행능력의 향상을 도모하고자 복막투석환자 간호업무 표준을 개발하는 방법론적 연구이다. 본 연구의 목적을 달성하기 위해 먼저 문헌고찰과 미국간호협회 와 미국신장간호협회 및 한국간호협회의 업무표준과 분야별 업무표준 개발 연구를 토대로 간호과정에 맞추어 우리나라의 임상상황에 적합한 활동들로 구성된 예비문항을 작성한 후 초점 집단의 검토를 거쳐 복막투석환자 간호업무 표준(안)을 개발 하였다. 개발된 업무 표준(안)은 3차 진료기관 복막 투석실에서 3년 이상 업무를 수행한 간호사 10명으로부터 내용의 타당도를 검증 받은 후 4개의 항목을 수정, 보완하였다. 마지막으로 2,3차 진료기관의 20개의 복막 투석실에서 간호업무를 직접 수행하고 있는 실무자로부터 수행정도를 조사하여 실무적합성을 확인 하고 최종적으로 완성된 복막투석환자 간호업무 표준으로 개발하였다. 본 연구의 결과는 다음과 같다. 1. 복막투석환자 간호업무 표준 개발의 예비 문항을 간호과정 적용하여 실무위주로 입원에서 퇴원, 추후관리 순서로 작성하였다. 2. 개발된 업무 표준(안)은 12개의 표준과 32개의 기준 및 53개의 지표, 총 254개의 간호활동으로 구성되었다. 3. 전문가 집단의 내용 타당도 검증은 업무 표준(안)의 각 항목에 대하여 4점 척도를 사용하여 3점 ‘타당하다’, 4점 ‘매우 타당하다’에 동의한 전문가수로 내용 타당도 지수 CVI(content validity index)구한 결과 합의율 80% 이상인 것을 취하였고 전체 항목에 대한 CVI는 0.92로 높게 나타났다. 전문가 합의율이 80%미만인 항목은 4개였다. 4. 개발된 복막투석간호 업무표준(안)에 대한 실무적합성을 확인하기위해 임상에서의 수행여부를 조사한 결과 전체 업무 수행율은 88.4%로 높게 나타났다. 5. 최종적으로 개발된 복막투석환자 간호업무 표준은 자료 수집, 간호 진단, 계획, 수행, 평가와 전문직 성과표준인 윤리, 업무수행 평가, 교육, 연구, 협동, 자원 활용, 간호의 질로 12개의 표준과 이에 따른 32개의 기준, 53개의 지표, 254개의 간호활동으로 구성되었다. 본 연구를 통해 개발된 복막투석환자 간호업무 표준은 앞으로 임상실무에 본격적으로 적용한 후 더욱 정렬화 시킨다면 복막투석환자를 간호하는 간호사들의 질 평가를 위한 제도적 장치로써 사용될 수 있을 것이며, 업무지침서, 업무수행 능력을 향상시키기 위한 도구로도 널리 활용될 수 있을 것으로 사려 된다.;This study is a methodological research to provide fundamental data for qualitative evaluation of peritoneal dialysis patients and to develop peritoneal dialysis nursing practice standards for improved clinical practice performance abilities As a way of research, preliminary questions with regards to practices reflecting Korea's unique clinical situation were selected. They are based on literature review, practice standards set by ANA(American Nurses Association), American Kidney Nurses Association and Korea Nurses Association and disciplinary practice standards development research. And after a review by a focus group, peritoneal dialysis nursing practice standards were developed. Four of the standards were modified and upgraded after 10 nurses with more than 3 years of real experiences in peritoneal dialysis unit of tertiary care providers verified the content validity of standard. And the final version of standards were developed after working-level nursing staff in 20 peritoneal dialysis unit of secondary and tertiary care providers researched the standards performance level and confirmed their adequacy for practice. The results of this study are as follows. 1. The preliminary set of standards of CAPD nursing was established in accordance with the nursing process of admission, discharge and follow-up. 2. The developed standards of practice of 12 standards, 32 criteria, 53 indicators, and 254 nursing activities. 3. The CVI(content validity index) was obtained by the professional who answered 3(relevant)and 4(highly relevant)on the 4-point scale. Activities with more than 80% content validity were selected, and their CVI was 0.92. Four nursing activities showed less than 80% content validity. 4. A survey was conducted to assess the adequacy of the developed standards of CAPD nursing. It showed that 88.4% of the questioned nursing activities are being performed in actual clinical environment. 5. The final version of the standards is comprised of 12 standards, and their subsets of 32 criteria, 53 indicator, and 254 nursing activities. The 12 standards were set based on two perspective; standards of nursing practice and those of professionalism. The former includes data collection, nursing diagnosis, nursing plan, nursing performance, evaluation; the latter includes ethics, performance evaluation, education, research, cooperation, resource utilization and quality of nursing. The developed set of nursing standards for peritoneal dialysis patients, with modification and upgrades, are to be used as an institutional measure for qualitative evaluation of the nursing practice and as a referential device for enhanced clinical practice.
