Abstract
과민성 장 증후군은 전 세계적으로 높은 유병률을 나타내고, 증상이 있더라도 대다수가 적극적으로 치료를 받지 않고 있다. 아직 명확한 원인과 치료법이 밝혀지지 않았지만 정신적 및 심리적 요인과 관련이 깊은 것으로 알려져 있는데 간호사라는 직업은 업무 중 심리적 스트레스를 많이 받는 직업이다. 본 연구는 간호사를 대상으로 과민성 장 증후군의 유병률, 과민성 장 증후군과 삶의 질과의 관계, 그리고 과민성 장 증후군과 정신건강과의 관계를 파악하기 위하여 시도되었다. 본 연구는 서울시내의 4개 종합병원에 근무하는 간호사에게 구조화된 설문지를 직접 작성하게 한 후 수거하는 방법으로 총 587명을 대상으로 하였다. 연구도구는 과민성 장 증후군을 분류하기 위하여 ROME Ⅱ를, 삶의 질을 측정하기 위하여 SF-36을, 정신건강을 측정하기 위하여 한국판 간이정신검사도구(SCL-90R)를 사용하였다. 수집된 자료는 SPSS WIN 12.0 Program으로 전산처리하였다. 본 연구의 결과는 다음과 같다. 1. 본 연구의 대상자 중 과민성 장 증후군으로 분류된 대상자는 208명으로 간호사의 과민성 장 증후군에 대한 유병률은 35.4%로 나타났다. 2. 일반적 특성에 따른 과민성 장 증후군의 유병률은 결혼 상태(X²=9.115, p=.010), 임상경력(X²=8.740, p=.033), 근무부서(X²=14.427, p=.044)에 따라 통계적으로 유의한 차이를 보였다. 그리고 과민성 장 증후군 대상자 중 지난 1년 동안 복통 혹은 복부불쾌감으로 의료기관을 방문한 대상자는 31.2%, 복통 혹은 복부불쾌감으로 인하여 약물복용을 한 대상자는 69.2%이었다. 3. 과민성 장 증후군 대상자의 삶의 질(SF-36)의 점수는 과민성 장 증후군의 증상이 없는 대상자에 비하여 낮았는데, 신체적 기능(t=3.284, p=.001), 사회적 기능(t=4.396, p=.000), 신체적 역할제한(t=3.265, p=.001), 감정적 역할제한(t=4.435, p=.000), 정신건강(t=4.057, p=.000), 활력(t=3.775, p=.00), 통증(t=5.807, p=.000), 일반건강(t=4.453, p=.000)의 8가지 항목 모두에서 통계적으로 유의하게 낮은 수준으로 나타났다. 즉, 과민성 장 증후군 대상자의 삶의 질이 과민성 장 증후군의 증상이 없는 대상자에 비하여 낮았다. 4. 과민성 장 증후군 대상자의 정신건강(SCL-90R)의 점수는 과민성 장 증후군의 증상이 없는 대상자에 비하여 높았는데, 신체화(t=-6.121, p=.000), 강박증(t=-5.181, p=.000), 대인예민성(t=-4.061, p=.000), 우울(t=-3.867, p=.000), 불안(t=-5.627, p=.000), 적대감(t=-3.721, p=.000), 공포불안(t=-2.697, p=.007), 편집증(t=-3.527, p=.000), 정신증(t=-3.997, p=.000)의 9가지 증상차원에서 모두 통계적으로 유의하게 높은 것으로 나타났다. 즉 과민성 장 증후군 대상자의 정신건강은 과민성 장 증후군의 증상이 없는 대상자에 비하여 해를 입은 것으로 나타났다. 이상의 연구에서 간호사의 과민성 장 증후군의 유병률은 35.4%이었으며 과민성 장 증후군은 삶의 질과 정신건강에 부정적인 영향을 미친다는 것을 알 수 있었다. 따라서 간호사를 대상으로 한 과민성 장 증후군에 대한 교육이 필요하고 그들의 삶의 질을 향상시키고 그들이 심리적 안정을 취할 수 있는 방안을 모색하여야 할 것이다.;The purpose of this study was to survey rate of nurses' IBS affections and analyze the relationships between IBS and their quality of life, and between IBS and their mental health. For this purpose, 587 nurses working for 4 general hospitals in Seoul were sampled. In order to classify their IBS, measure their quality of life and mental health, ROME Ⅱ, SF-36 and SCL-90R were used, respectively. The collected data were processed using the SPSS WIN 12.0 program. The results of this study can be summarized as follows; 1. 208 nurses(35.4%) of the sample nurses were found to suffer from IBS. 2. The rate of nurses' IBS affections differed significantly depending on such demographic variables as marital status(X²=9.115, p=.010), clinical career(X²=8.740, p=.033) and working area(X²=14.427, p=.044). And 31.2% of the nurses had visited the medical institutions for abdominal pain or discomfort for the last year, while 69.2% of them took medicine for abdominal pain or discomfort. 3. IBS group scored lower SF-36 points than non-IBS group. The lower SF-36 points were statistically significant in all sub-areas(8 scales): physical functioning(t=3.284, p=.001), social functioning (t=4.396, p=.000), role-physical limitation(t=3.265, p=.001), role -emotional limitation(t=4.435, p=.000), mental health (t=4.057, p=.000), vitality(t=3.775, p=.000), bodily pain (t=5.807, p=.000) and general health(t=4.453, p=.000). Namely, the IBS group's quality of life was poorer than non-IBS group's. 4. IBS group scored higher SCL-90R points than non-IBS group. The higher SCL-90R points were statistically significant in all sub-areas(9 symptom dimensions): somatization(t=-6.121, p=.000), obsessive-compulsive(t=-5.181, p=.000), interpersonal sensitivity (t=-4.061, p=.000), depression(t=-3.867, p=.000), anxiety(t=-5.627, p=.000), hostility(t=-3.721, p=.000), phobic anxiety(t=-2.697, p=.007), paranoid ideation(t=-3.527, p=.000) and psychoticism(t=-3.997, p=.000). Namely, the IBS group's mental health was poorer than non-IBS group's. As discussed above, 35.4% of the nurses were suffering from IBS, and IBS affected their quality of life and mental health. Therefore, nurses need to be educated on IBS and some measures need to be taken to improve their quality of life and stabilize them psychologically.
