성매매 현장에서의 경험이 탈성매매 여성의 정신건강에 미치는 영향에 관한 연구

Abstract

본 연구에서는 탈성매매 여성들의 인구사회학적 특성과 성매매 현장에서 경험하는 사건의 종류 및 그 정도를 밝히고 그러한 사건들이 성매매 여성들이 정신건강에 어떠한 영향을 미치는지 밝히고자 한다. 이를 위해 연구자는 성매매 현장에서 일어나는 사건들을 폭력, 신체적 손상, 사회적 손상, 법적 처분, 정서적 손상으로 범주를 나누어 살펴보았다. 본 연구는 전국 38개 중 사전 전화조사를 통해 설문에 참여할 의사가 있는 16개 성매매피해여성지원시설에서 보호를 받고 있는 탈성매매여성들을 대상으로 우편설문을 실시하였다. 총 회수된 설문지는 87부이며 이중 부실한 응답으로 통계분석에 적합하지 않은 설문지 8부를 제외한 79부의 설문지가 통계 분석에 사용되었다. 주요 연구 결과는 다음과 같다. 첫째, 탈성매매 여성들은 정신건강 전체지수와 9개의 하위영역별로 성매매 현장의 폭력, 신체적 손상, 사회적 손상, 법적 처분, 정서적 손상을 경험한 여성이 그렇지 않은 여성에 비해 정신과적 증상이 높은 것으로 나타났다. 둘째, 성매매 관련 특성인 성매매 유입 이전의 성폭력 경험, 가출경험, 성매매 유입 연령, 유입 기간, 선불금 유무, 선불금 금액에 따라 정신건강수준에 차이가 있는지 살펴보았다. 분석 결과 성매매 위험요인인 성매매 이전 성폭력 경험을 가진 여성이 그렇지 않은 여성에 비해 정신건강 수준이 낮음을 알 수 있었다. 또한 성매매 유입 후 선불금이 있는 여성이 그렇지 않은 여성에 비해, 선불금 금액이 많은 여성들이 그렇지 않은 여성들에 비해 낮은 정신건강수준을 보였다. 셋째, 성매매 현장에서 경험한 사건들과 성매매 관련 특성, 그리고 정신건강수준 간의 상관관계를 분석하여 유의미한 상관관계를 나타내는 변수들을 선별할 수 있었다. 성매매 관련 특성에서는 선불금과 성매매 유입 이전 경험한 성폭력이 정신건강수준과 유의미한 상관관계를 보이는 것으로 나타났다. 넷째, 성매매 현장에서 경험한 사건들 중 정신건강수준에 영향을 미치는 요인을 분석하기 위하여 로지스틱 회귀분석을 실시하였고, 분석 결과 신체적 손상과 윤락 전과, 성매매 유입 이전 성폭력 경험 등이 정신건강에 영향을 주는 것으로 나타났다. 본 연구를 통해 얻을 수 있는 사회복지적 함의는 다음과 같다. 첫째, 성매매 현장에서 일어나는 사건들을 범주화 하여 이에 대한 자료를 마련함으로서 성매매 피해여성들이 경험하는 신체적, 정서적, 사회적 손상의 유형과 정도를 제시하였다. 둘째, 탈성매매 여성의 정신건강에 영향을 미치는 요인을 분석함으로서 탈성매매 여성들에 대한 임상적 개입에 활용할 수 있는 기초 자료를 마련하였다. 마지막으로 본 연구의 한계점을 극복하기 위한 후속 연구를 위한 제언을 하면 다음과 같다. 첫째, 전국의 시설 38개 중 16개의 시설을 대상으로 조사한 연구이므로 연구 결과 탈성매매 여성에 대해 일반화시키기 위하여 성매매피해여성지원시설의 전수를 포함한 연구가 이루어져야 할 것이다. 둘째,본 연구에서 성매매 현장에서 경험하는 사건 유형 중 폭력의 범주에 대한 후속 연구가 필요하다.선행연구와는 달리 폭력이 정신건강에 영향을 미치지 않는 것으로 나타나 이에 대한 연구가 이루어져야 할 것이다. 셋째,탈성매매 여성들의 정신건강을 연구함에 있어 보다 다각적인 접근을 시도할 필요가 있다. 넷째,본 연구에서는 성매매 현장의 경험 유형에 따라 다섯 가지의 범주를 구분하여 각 문항이 하나의 사건을 대표할 수 있도록 구성하였다.이를 기반으로 후속연구에서는 성매매의 특성을 반영할 수 있는 척도 개발이 필요할 것이다.;The purpose of this study was to examine kinds of event that occurs in prostitution spot and to clear how these events effect on mental health of victims of prostitution. For this research, events that occur in prostitution spot are categorized five areas; violence, physical injury, social loss, legal punishment, emotional injury. And for measurement of mental health, SCL-90R was used. This study was based on the survey of 79 victims of prostitution from 16 assistance facilities, and the sruvey was conducted from Oct. 2004 to Nov. 2004. For the purpose of the study, descriptive statistical analysis, t-test, ANOVA, correlation, and logistic regression were used. The results were as follows. First, victims of prostitution who experienced violence, physical injury, social loss, legal punishment, emotional injury brought out higher psychiatric symptoms more than the others. Second, in the analysis of prostitution related variables such as sexual abuse or run away experience before entering prostitution, entering age and period of prostitution, debt bondage, women who were sexually abused before prostitution were identified as significant variables affecting mental health. And women who have debt showed psychiatric symptom. Third, through correlation analysis between events and mental health, between prostitution related variables and mental health, significant variables were selected. Significant variables affecting mental health were sexual abuse before prostitution, entering age and period of prostitution and event of pregnancy, medical treatment, genital disease, STD, prostitution criminal record and abnormal sexual activity. Forth, to analysis factors that affect mental health, logistic regression was tested and factors such as sexual abuse before prostitution, medical treatment, genital disease, STD, prostitution criminal record influenced mental health. Based on the results of the study, implications for social work practices were discussed. Although participants of this study was limited to 16 assistance facilities, this study presented data about events that victims of prostitution experienced. Also this study provided basic data about factors that influence mental health of victims of prostitution.
