암환자를 돌보는 보호부담이 가족원의 우울에 미치는 영향에 관한 연구

Abstract

암환자가 경험하는 심리사회적인 문제점들과 이를 감소시키기 위한 사회적 지지, 가족적 스트레스에 관한 연구들이 진행되고 있으며 최근에는 암환자가족의 보호부담, 스트레스, 가족기능 등에도 관심을 기울이고 있다. 그러나 아직까지 암환자 가족에 대한 연구들은 극히 제한적으로 이루어져왔다. 암환자의 돌봄으로 인한 가족원의 신체적, 정신적 어려움은 가족원의 신체적, 정신적 건강에 부정적인 영향을 미치게 될 것이며, 질병으로 인해 변화된 새로운 환경에 적응하지 못하는 결과를 가져올 것으로 보인다. 따라서 본 연구에서는 암환자를 돌보는 가족들이 경험하게 되는 보호부담감이 가족의 우울감에 미치는 영향을 파악하여 암환자 가족의 보호부담과 우울을 줄이며 새로운 치료환경에 잘 적응할 수 있는 의료사회사업의 개별상담 및 집단상담 프로그램을 위한 기초자료를 제공하는데 그 목적을 두었다. 본 연구를 위해 현재 대학병원 이상의 병원에서 치료중인 암환자 가족들 총 132명을 대상으로 설문조사를 실시하였으며 암환자 가족이 경험하는 보호부담과 우울감의 정도, 보호부담과 우울감의 관계를 알아보기 위하여 빈도분석, 상관분석, 위계적 회귀분석 등을 실시하였다. 본 연구의 구체적인 의의는 다음과 같다. 첫째, 이전까지 많은 분야의 연구에서 암환자에 대한 많은 관심은 가져왔으나 암환자를 돌보는 가족원이라는 대상으로 집중적인 연구가 이루어졌다는 것에 의의가 있다. 둘째, 암환자를 돌보는 가족이 경험하는 부정적인 경험인 보호부담감 뿐만 아니라 우울감에 대한 연구를 통하여 보호부담감이 가족의 우울에 미치는 영향력에 대한 부분을 고찰하였다. 연구결과는 다음과 같다. 첫째, 암환자 가족은 환자를 돌보는 과정에서 경험하고 있는 부담감은 중간값보다 높은 중증도 이상의 보호부담을 경험하고 있는 것으로 나타났다. 또한 암환자 가족의 보호부담감을 영역별로 살펴보면 경제적 부담감을 가장 높게 나타났으며 시간-의존적 부담감, 신체적 부담감, 발달적 부담감, 사회적 부담감 순으로 나타났다. 둘째, 암환자를 돌보는 가족원의 우울감 정도는 한국판 우울척도 표준화 연구결과에 따라 살펴보면 연구 대상자의 약 45.5%가 우울경향, 우울증, 심한 우울증을 보이는 것으로 나타났다. 셋째, 다른 변수를 통제한 상태에서도 암환자 가족의 보호부담감이 가족의 우울감에 영향을 미치는 중요한 변수로 나타났으며, 이는 보호부담감이 증가할수록 우울감이 증가한다는 것을 알 수 있다. 이와 같은 결론을 바탕으로 다음과 같은 제언을 하고자 한다. 첫째, 질병단계에 따른 암환자 및 가족의 어려움이 다양하게 나타날 수 있기 때문에 질병단계에 따를 보호부담감 및 우울감에 대한 후속연구가 필요할 것으로 보인다. 둘째, 암은 생명과 삶에 치명적인 영향을 미치고 있으나, 암이 발생한 부위에 따른 질병의 특성으로 인해 환자와 가족이 경험하는 심리사회적인 어려움 등은 차이가 있을 것으로 보여 이에 대한 심층적인 연구가 필요할 것으로 보인다. 특히 치료과정 중의 하나인 수술을 통해 환자의 신체 이미지에 가장 부정적인 영향을 미치며 외형적인 변화를 가져오는 유방암과 대장?직장암(장루)은 환자와 가족의 일상적인 삶에 많은 영향을 미치는 것으로 예상된다. 이는 암이라는 질병 자체가 가지고 있는 특성과 신체의 외형적 변화를 동시에 경험하게 됨에 따라 다른 암환자 및 가족과는 차별화 된 심리사회적인 특성에 중점을 둔 연구가 이루어져야 할 것이다.;This study examined the influence of depression on care burden for cancer patient's family to search for the intervention methods to promote the medical social work services. Also, this study examined the degree of the care burden and depression of cancer patient's family. Data were drawn from a self-administered questionnaire study. The questionnaires were answered by 132 family members who primary cared patient with cancer in university hospital. The frequency analysis, correlation, hierarchical multiple regression analysis were used for analysis of results. The major findings of the study were as follows: First, family members who primary cared cancer patients experienced a high level of care burden more than a mean score. Among the care burdens classified by 6 categories, financial burden was highly reported by family members of cancer patients. Second, about 45.5% family members of cancer patients who took part this study experienced subclinical, manifest, and severe depression. Third, between care burden and depression of cancer patient's family are related statistically. Among the burden, social burden of cancer patient's family was highly related on depression. Forth, in case of controlling other variables, care burden of cancer patient's family was a major factor that influenced depression. Base on these results, following implication can drawn out. First, because survival rate can influence all aspects of cancer patient's family, the study according to the degree of illness needs to concern. Second, cancer patients could be influence by their each cancer types. And also cancer patients and their family members may experience various difficulties according to the medical condition. As a results, the social work intervention for breast cancer and colon cancer patient who can have a negative body image after surgery and treatment and their family members is essential.
