Abstract
The Special Education Promotion Act Amendments of 2005 has included health impairments, such as heart diseases, kidney dysfunction, and liver diseases, as an independent special education category. Since then, students with chronic illnesses, who had experienced history of grade retention, academic achievement delays, and difficult peer relations due to long-term absenteeism, began to receive specialized support. The hospital school offers a continuum of educational services from the outset of hospitalization to discharge and to the outpatient clinics to enable these students to continue academic work and to provide psychological and emotional support while absent from school. Nevertheless, the best practices program as well as the efficient management model of the hospital school to meet the educational needs of the students with health impairments have not been developed. Since 2006, the hospital schools have been established at the national level and expanded throughout the nation, and more and more students with health impairments have asked for systematic educational support. Thus, there is a dire need to expedite the management support system of the hospital school to ensure quality education for students with health impairment. The purpose of this present study was two-fold. First, it was to investigate quality component factors of the management support system of the hospital school to improve educational, psychological and emotional services for students with health impairments. Second, this study was to validate the support system via field studies. To achieve this purpose, the following procedures were implemented. First, the preliminary items of the component factors of the hospital school management support system were developed by the author of this study, and these items were revised after being reviewed by two education professionals in the field of health impairments. In order to investigate content validity of the preliminary items, the adapted Delpi method was employed. More specifically, the content validation steps included as follow: Eight professionals rated the items individually. Based on the feedback received, the professionals were asked to rerate the items individually. To seek consensus, interactive discussion was conducted. Finally, the professionals were rated on an 11-Likert scale scores. Second, the social validity of the management support system of the hospital school was investigated by a total of 192 individuals, 123 professionals from various professional groups such as associations and education agencies, including 19 hospital school teachers, doctors, nurses, social workers, and hospital administrative support personnel, and 69 parents receiving services from hospital schools. These individuals were asked to respond about the importance and practice of the management support system of the hospital school on a 5-Likert scale scores. The questionnaire collected through the Delphi method was analyzed using technical statistics, and questionnaire collected through the survey was analyzed by frequency analysis, and the data were computed in frequency and percent. To investigate the difference on the hospital school operation types and the importance and practice of the management support system between groups, independent t-test and paired t-test were used, respectively. The results of this present study were as follow: First, the validated management support system of the hospital school was composed of 5 categories and 61 items, including systemizing human resource management and facilitation of professional collaboration(15 items), establishing family support system(7 items), efficient implementation of curriculum(21 items), streamlining administrative and financial management of the hospital school(10 items), and structuring educational environment(8 items). Second, the social validity results showed that all of the professional and parent groups acknowledged the importance of the 5 categories of the hospital school management support system. They also indicated ‘structuring educational environment’ as the most important category for students with health impairments. There was statistical difference between the parent groups and the professional groups in terms of the ‘establishing family support’ category. The parent groups indicated the 'establishing family support' category more important than the professional groups. Thus, there is a need for a collaborative approach to meet the family support needs. Third, the results about the importance and practice of the hospital school management support system rated by the professional groups of each hospital who were familiar with the hospital school operation system were as follow: There was no statistical difference on the importance of the hospital school management support system between the professional groups of the hospital school operated by the hospital and the education agency. For the practice of the hospital school management support system, the professional group of the hospital school operated by the education agency rated higher than the professional group of the hospital school operated by the hospital. Among 5 categories, the structuring educational environment category was rated as the most important in terms of both the importance and practice of the hospital school management support system. In the field of special education, educational supports for students with health impairments have recently been provided. There have not been obvious research outcomes and effective program development. Thus, it was meaningful for this study to investigate and validate the quality component factors of the hospital school management support system. More importantly, the component factors of the support system validated through this study provided the guidelines and standards for the educational policy development. The results of this study also provided the foundation for the further development in the area of this study and policy development.;2005년 특수교육진흥법 일부 개정을 통해 ‘심장장애·신장장애·간장애 등 만성질환으로 인한 건강장애’가 특수교육 대상 장애범주에 포함되면서 그동안 장기결석으로 인한 유급 문제, 학습지체 현상, 또래관계 유지의 어려움 등을 지녔던 만성질환 학생들이 특별한 교육지원을 받기 시작했다. 이와 같이 잦은 병원 입원이나 장기간 통원치료를 요하는 건강장애학생들이 병원에서도 지속적으로 공부하고 또래관계를 유지하는 등 심리·정서적 지원을 제공한다는 측면에서 병원학교는 건강장애학생에게 매우 중요한 교육지원 방법의 하나가 되고 있다. 하지만 병원학교 프로그램의 최상의 실제나 효율적인 병원학교 운영모델 등은 아직까지 개발되어 있지 않다. 최근 들어 병원학교가 전국적으로 확대 설치·운영되면서 점점 더 많은 건강장애학생들이 제도적으로 교육지원을 받게 되었으므로 보다 조기에 현장 적용 가능한 병원학교 운영 지원체계가 마련되어 건강장애학생의 질적인 교육을 보장할 필요가 있다. 따라서 본 연구에서는 병원학교가 건강장애학생의 학업의 연계성, 심리·정서적 안정 및 치료효과 증진을 위해 효율적으로 운영될 수 있도록 하기 위해 병원학교 운영 지원체계의 구성요소를 추출하고 수정된 델파이 기법 및 설문조사를 통해 지원체계를 타당화하고자 하였다. 본 연구에서는 건강장애학생을 위한 병원학교 운영 지원체계의 타당화라는 연구 목적을 달성하기 위해 먼저 문헌연구와 실증자료 분석을 통해 1차 개발한 병원학교 운영 지원체계 구성요소의 예비항목을 건강장애학생 교육 내용 전문가 2명의 검토를 받아 정리한 후 이에 대한 내용 타당도 검증을 위해 수정된 델파이 기법을 사용하였다. 즉 8명의 전문가가 개별 평정, 피드백을 통한 개별 재평정, 상호작용적 토론, 11점 척도에 의한 최종 평정 등의 단계를 거쳤다. 둘째 병원학교 운영 지원체계에 관한 사회적 타당도 검증을 위해 전국 19개 병원학교의 교사, 의사, 간호사, 사회복지사, 병원행정지원팀원, 그리고 협회 직원 및 교육청의 전문직 등 전문가 123명과 병원학교 이용 학생의 학부모 69명 총 192명을 대상으로 병원학교 운영 지원체계에 대해 5점 척도로 중요도와 실행도를 각각 조사하였다. 수정된 델파이 절차에서 수거된 질문지는 기술통계로 분석하였고, 설문조사를 통해 수거된 질문지는 빈도분석을 통해 반응 수와 백분율을 산출하였으며, 병원학교 운영 형태별로 집단 간 및 중요도와 실행도 인식 간 차이를 검증하기 위하여 독립표본 t검정(independent t-test)과 대응표본 t검정(paired t-test)을 각각 사용하였다 본 연구를 통해 얻어진 결과는 다음과 같다. 첫째, 본 연구 결과 타당한 것으로 최종 정리된 병원학교 운영 지원체계는 인적자원 관리 체계화 및 전문가 간 협력 촉진 15개 항목, 가족지원 체제 구축 7개, 교육과정 운영의 효율화 21개, 병원학교의 행·재정적 운영체계의 효율화 10개, 교육 환경 구성 8개 등 총 5개 범주 61개 항목으로 구성되었다. 둘째, 병원학교 운영 지원체계 구성요소에 대한 사회적 타당도 검증 결과 전문가와 학부모 전체 집단은 물론 각 집단별로도 병원학교 운영 지원체계의 5개 범주 모두를 중요하게 인식하는 것으로 나타났다. 5개 범주 중 ‘교육 환경 구성’은 모든 집단에서 건강장애학생을 위한 병원학교 운영 지원체계 구성요소로서 가장 중요하다고 인식하였다. ‘가족지원 체제 구축’의 중요 정도에 대해서는 학부모 집단이 전문가 집단보다 더 중요하게 인식하고 실행도는 학부모 집단이 더 낮게 인식하는 것으로 나타났다. 따라서 전문가들은 건강장애학생 가족의 지원 요구에 대해 관심을 갖고 협력적으로 접근해 나갈 필요가 있음을 알 수 있었다. 셋째, 각 병원학교의 운영 체계를 보다 잘 아는 전문가 집단만을 대상으로 병원학교 운영 형태에 따라 병원학교 운영 지원체계의 중요도와 실행도에 대한 인식을 조사한 결과, 교육청 소속 병원학교 전문가 집단이 병원 소속 병원학교 전문가 집단보다 병원학교 운영 지원체계 각 범주에 대해 좀더 중요하게 인식하였지만, 두 집단 간에 유의미한 차가 없는 것으로 나타났다. 병원학교 운영 지원체계의 실행도에 대한 인식에 있어서는 5개 범주 중 교육과정 운영의 효율화 범주에 있어서만 교육청 소속 병원학교 전문가가 병원 소속 병원학교 전문가보다 유의미하게 더 잘 실행되는 것으로 인식하였다. 교육 환경 구성 범주의 경우 중요도와 실행도 모두 가장 높은 수준으로 인식됨을 알 수 있었다. 본 연구는 아직까지 건강장애학생을 위한 교육지원 분야에서 뚜렷한 연구 성과나 프로그램이 개발되지 않은 현 시점에서 지원체계의 질적 구성요소들을 파악하여 건강장애나 병원학교 운영 관련 전문가와 현장 전문가 및 학부모들을 대상으로 타당화 과정을 거침으로써 실제로 적용 가능한 지침을 제공한다는 점에서 큰 의의를 지닌다고 볼 수 있다. 특히 연구 결과 도출된 지원체계의 구성요소들은 프로그램 개발 내용들과 정책적 차원에서 개발해야 하는 지침과 기준을 제시하고 있어 향후 이 분야의 학문과 정책 발전에 중요한 기초를 마련하였다고 본다.
