dc.description.abstract약물의 개발은 성인을 중심으로 이루어지고 있기 때문에, 과거에 미국에서 성인에게 처방되고 있는 약물의 약 80% 정도가 소아 환자들에게 off label로 사용되었다. 성인용으로 허가된 약물을 unlicensed 또는 off label로 소아에게 처방하는 경우, 적정한 용량을 투여하기가 어렵고 대부분 조제상의 어려움도 크다. 소아의 약력학적 및 약동학적 특징이 성인과 다르기 때문에 약효를 제대로 기대할 수 없거나 예측하지 못한 연령 특이적 이상반응이 나타날 수도 있다. 미국 및 유럽연합은 소아에게 적정한 약물을 사용하기 위하여 국가적인 노력을 기울이고 있지만, 한국에서는 그 노력이 아직 미미하다. 본 연구의 목적은 병원에서 소아 입원환자에게 발급되는 처방전을 토대로 첫째 약물의 사용 양상을 분석하여 소아환자에서의 투약 상의 문제점 등을 찾아보고, 둘째 소아용 소용량의 정제나 시럽제제가 필요한 약물을 살펴보며, 셋째 소아에게 unlicensed 또는 off label로 약물이 처방되는 비율을 조사하여 소아에 대한 약물의 적절한 사용을 위한 방안 마련에 기여하고자 하는데 있다. 전국의 총 22개 병원으로부터 2003년 2월 중 하루의 소아 입원환자의 처방전을 제공받아 병원 별 및 병상규모 별(1000병상 이상, 500병상 이상 1000병상 미만, 500병상 미만)로 ‘각 연령대의 조사 환자수 및 정제나 캡슐제의 복용이 어려운 5세 이하 환자의 비율’, ‘환자 당 처방약품 수’, ‘단위 제형 당 1 미만의 용량으로 처방되고 있는 경구고형제(일반명)와 각각의 단위 제형 당 1회 투여량’, ‘경구고형제의 단위 제형 당 0.5미만 사용비율, 1미만 사용비율, 1 및 2이상을 제외한 사용 비율 및 0.5, 1, 1.5 및 2이상을 제외한 사용비율’, ‘경구고형제 중 단위 제형 당 1회 투여량이 0.5미만인 용량으로 2회 이상 사용된 약품을 대체할 수 있는 시럽제나 소용량의 경구 고형제 국내 존재 유무’, ‘경구고형제가 unlicensed 또는 off label로 사용되는 비율’을 평가하여 다음과 같은 결과를 얻었다. 1. 병상 수 1000이상의 5개 병원에서 총 767명, 병상 수 500이상 1000미만 의 12개 병원에서 총 982명, 병상 수 500미만의 5개 병원에서 총 243명의 환자가 이번 조사에 포함되었다. 정제나 캡슐 제형의 약물을 그대로 복용하기 어려우므로 일반적으로 시럽제 등 별도의 소아용 제형을 필요로 하는 5세 이하의 환자는 각 병상구분 별로 63.0%, 57.9%, 86.0%였다. 2.분쇄하여 투여하면 원 제형의 제제학적 특징을 상실하여 기대하는 약효를 얻기 어려운 서방정(Feroba-U, Vaproate), 장용정(Medilac-S), 다층정 (Bease)이나 분쇄하여 균등한 조제가 힘든 연질캡슐(Nifedifine, Toco-pherol)도 단위 제형 당 1미만의 용량으로 처방되고 있었다. 3.단위 제형 당 0.5미만의 경구고형제를 사용한 비율은 병상 수 1000이상의 병원에서는 29.9%, 병상 수 500이상 1000미만의 병원에서는 36.5%, 병상 수 500미만의 병원에서는 60.8%였으며, 1미만을 사용한 비율은 각각 54.3%, 60.6%, 81.6%로 병상규모가 작아질수록 현저하게 그 비율은 높아졌다. 5세 이하 소아환자 수의 차이에 비해 그 비율의 차이가 큰 것은 병원에서 구비하고 있는 약물의 수 등 다른 요인이 작용하고 있을 것으로 보인다. 병상 수 1000이상인 병원에서는 원내 제제도 만들어 사용하고 있었다. 4.단위 제형 당 1미만의 용량으로 다빈도 처방되고 약물은 Bromhexine, Folic acid, Spironolactone, Furosemide, Mequitazine, Domperidone, Streptokinase/Streptodornase, Chloropheniramine, Urodeoxycholic acid (UDCA), Fenoterol 등 이었다. 이들 약물 중 Furosemide, Mequitazine, Domperidone, Chloropheniramine은 시럽제가 있음에도 정제로 처방된 약물의 예이다. 5.경구고형제 중 0.5단위용량 미만으로 2회 이상 사용된 약물 총 63품목 중 34품목(54.0%)이 시럽 또는 소용량의 정제나 캡슐제가 유통되고 있음 에도 처방되었으며, 시럽제가 있는 경우도 24품목(38.1%)이나 되었다. 6.병상 수 1000이상의 4개 병원이 경구 고형제를 unlicensed 또는 off label로 사용한 비율은 34.2%(각각 36.2%, 37.9%, 36.5%, 28.2%)였다. 종합해 보면, 소아는 적정한 용량의 약물을 복용하거나 효능 및 안전성이 보증되는 약물을 복용하기에는 힘든 현실에 처해 있다. 신생아나 유아처럼 연령이 어릴수록 약물에 예민하지만 이러한 위험의 가능성은 더 크다. 해당 성분의 사용이 허가되었다 할지라도 대부분의 경우 명확한 용량 지침이 없이 ‘연령에 따른 적의증감’ 표시가 있거나 소아에게 적합한 제형이 없다. 그러므로, 자세하게 적응증 및 제형을 따져서 조사한다면, unlicensed 또는 off label로 사용된 경구고형제의 비율은 34.2%보다 훨씬 높을 것으로 예상된다. 소아에 대한 명확한 labeling은 단계적으로 소아임상시험의 확대, 시판 후 조사의 활용 등으로 정부와 제약회사가 협력하여 함께 풀어야 할 과제이며, 소아용 제제의 개발 및 생산을 장려하는 정책이 있어야 할 것이다. 또, 우선은 병원약사회나 대한약사회 차원에서, 적합한 제형이 없어 산제 또는 시럽제로 불가피하게 조제해야 하는 약물 중 다빈도 또는 배산제, 용액 등의 선택 등이 까다로운 약물을 우선 선정한 후 조제 지침을 마련하여 일선 약사에게 정보를 제공하는 것도 바람직할 것이다. 대체약이 있음에도 불구하고 경구고형제의 분쇄 투여와 같은 잘못된 처방 및 조제관행을 없애기 위해서 정부차원에서의 정책이나 캠페인 등도 있어야 할 것이다.;Since most pharmaceutical products are developed with adults as the main target, 80% of drugs prescribed to adults in the United States were not labeled for use in children in the past. When these drugs are used off-label in children, the patients may not benefit from adequate efficacy or may experience unexpected age-specific adverse reactions as it is difficult to determine the proper dosage for children and also because the pharmacodynamic and pharmacokinetic characteristics of children differ from those of adults. The relevant authorities in the United States and the European Union have made it a national agenda to use proper drugs in children, but such efforts are not yet significant in Korea. The objectives of this study is to examine the prescriptions written for hospitalized pediatric patients so as to investigate problems related to their drug administration and to identify pharmaceutical products that need to be developed into low-dosage tablets or syrups for pediatric use. This study is also intended to contribute towards the development of proper prescription practices in Korea by investigating the name and usage rates of pharmaceutical products that are not approved for pediatric use but are nonetheless being administered to children. Pediatric prescriptions for one pre-determined day in February 2003 were collected from a total of 22 hospitals in the nation and analyzed by hospital or hospital groups classified by the number of beds (1,000 beds or more, 500 or more but under 1,000 beds, less that 500 beds) according to the following criteria: ‘number of patients by age brackets and the proportion of patients under age five who may have difficulties taking tablets or capsules’, ‘number of drugs prescribed per patient’, ‘name(generic name) of oral solid formulations and amount of drug taken per administration for each formulation unit, prescription frequency for each administration dose per formulation unit (usage rate per unit of solid formulation for the following proportions: less than 0.5; less than 1; exclusion of 1, 2 and above, exclusion of 0.5, 1, 1.5, 2 and above)’, ‘local availability, in syrups or smaller-dosage oral solid formulations, of oral solid formulations that have been administered in proportions of less than 0.5 per formulation unit two or more times’, ‘rate of off-label or unlicensed use of oral solid formulations in Korea.’ The following results were obtained. 1.A total of 767 patients, 982 patients, and 243 patients from five hospitals with 1,000 or more beds, 12 hospitals with 500 or more but less than 1,000 beds and five hospitals with less than 500 beds, respectively, were studied in the course of this study. The percentages of patients, to five years of age generally have difficulties taking medications in the form of tablets or capsules and require syrup or special pediatric pills, in these hospital groups were 63.0%, 57.9%, and 86.0%, respectively. 2.Drugs whose efficacies are undermined when the original formulations are compromised include sustained release tablets (Feroba-U, Valproate), enteric-coated drugs (Medilac-S), and multi-layered tablets (Bease). Liquid filled capsules (Nifedifine, Tocopherol) whose contents are not equally distributed to each wanted quantity when crushed. However, these drugs were being prescribed in proportions of less than 1 per formulation unit. 3.The usage rates in the proportion of less than 0.5 per unit of oral solid formulation were 29.9%, 36.5%, and 60.8%, respectively, in hospitals with 1,000 or more beds, those with 500 or more but less than 1,000 beds, and hospitals with less than 500 beds. The usage rates in the proportion of less than 1 per unit were 54.3%, 60.6%, and 81.6%, respectively, showing a significant increment as the number of beds decreased. Though the number of pediatric patients under five years of age may be higher in the hospital group of less than 500 beds, the percentage difference is much more significant than the difference in patient numbers. Thus, it is likely that other factors are in play, such as the number of drugs in the hospital formulary. Hospitals with 1,000 or more beds also used formulations compounded within the hospital. 4.Drugs most frequently crushed for use include Bromhexine, Folic acid, Spironolactone, Furosemide, Mequitazine, Domperidone, Streptokinase/ Streptodornase, Chloropheniramine, UDCA and Fenoterol. Among these drugs, Furosemide, Mequitazine, Domperidone and Chloropheniramine were available as the formulation of pediatric syrups in the market but were prescribed as the formulation of tablets. 5.Of the 63 oral solid formulation products that were used two or more times in the proportion of less than 0.5 units, 34 products (54.0%) were used as such despite the fact that syrups and smaller-dosage tablets or capsules were available in the market, and 24 products (38.1%) so even when syrup formulations were available. 6.An investigation of four hospitals with 1,000 or more beds revealed that the percentage of oral solid formulations used an unlicensed or off label manner in children was 34.2% (36.2%, 37.9%, 36.5%, and 28.2%, respectively, in the four hospitals). Overall, pediatric patients are faced with the reality in which it is difficult to ensure that they are administered with the appropriate amount of medications, not to mention drugs with their efficacy and safety guaranteed. The youngest of pediatric patients such as the new-borns and infants are apt to be more sensitive to drug effects, therefore are exposed to even greater risks. Active substances that are approved for pediatric use often lack precise information on proper dosage (only labeled with ‘modify dose according to age’) or do not come in formulations suitable for pediatric administration. Therefore, the actual rate of off-label use of drugs in children would be much higher than the 34.2% obtained in the study if rigorously investigated by indication and formulation. Proper labeling of medication for pediatric use is a challenge that must be faced together by both the government and the pharmaceutical industry. Various means including stepwise expansion of pediatric clinical trials and post marketing surveillances would be useful. Policies to encourage development and manufacture of the formulations for pediatrics are needed. Meanwhile, it would be desirable that the Korean Society of Health-System Pharmacists or the Korean Pharmaceutical Association identify, among drugs that inevitably have to be crushed to powders or syrups for use in children, the most frequently crushed drugs or those that require special attention in choosing dispensing powders or solutions and develop dispensing guidelines that can be adopted by pharmacists in practice. Moreover, government-led policies and campaigns are needed to correct unsound prescription and dispensing practices such as using crushed forms of certain oral solid formulations when alternative formulations are available in the market.-
