Abstract
The purpose of this research was to investigate in-patients' perception on foodservice and examine their consumption of meals serviced at hospitals. The study consisted of two parts - an in-patient survey and a meal consumption measurement. Three general hospitals with over 400 beds in Seoul and Chon-An agreed to participate in the research. In Study I, 516 in-patients of the hospitals were surveyed on their consumptions of meals, reasons of plate wastes if any, perceptions of foodservice quality, and demographic information. A response rate was 76% after excluding responses with significant missing data. In Study II, their consumption of the meals provided at the hospitals was examined using an aggregate selection plate waste measurement technique. The food intakes were measured for 3 days: 2 weekdays and 1 of weekends. Statistical analyses were performed using SPSS Windows (ver 12.0) program. The results are summarized as follows: In Study I, the patients perceived that they consumed 60-70% of the meals provided. No significant difference was found in consumption between regular diets and therapeutic diets but the regular diet patients consumed more than the therapeutic diet patients. Seventeen percent of the regular diet patients and eleven percent of the therapeutic diet patients responded that they consumed all that they were provided. The patients who left plate wastes responded "loss of appetite" and "sick and tired" as major reasons for the plate waste. The patients perceived variety of menus, sanitation, and consistency of food taste in hospital foodservice poor. The perceptions of the therapeutic diet patients were significantly lower than those of the regular diet patients on food temperature (p<.05), consistency of food tastes (p<.01), and nutrition information (p<.001). There were no significant differences on sanitation, variety of menus, exact meal time, and kindness of servers. To increase patients' consumption of the meals at hospitals, dietitians should pay more attentions on maintaining proper food temperature and consistent food taste and improving sanitation. In Study II, an aggregate selection plate waste measurement revealed that the regular diet patients consumed 78.5% of the meals they were served and diabetes-diet and low sodium-diet patients consumed 81.6% and 77.1%, respectively. That is, 20-25% of the nutrients the patients were supposed to intake were not consumed. Even though the dietitians plan and provide meals based on the patients' conditions and nutritional requirements, significant amount of the nutrients are lost, which makes negative impacts on patients' recovery and well-being. To improve food service quality at the hospitals and achieve the goals of the foodservice, the dietitians should reduce the gaps between what they provide and what the patients actually consume. The dietitians and foodservice staff at the hospitals should plan menus with consideration of patients' food preferences and conditions as well as provide better environments where the patients can consume meals more. Since the therapeutic diet patients are interested in information on their meals and nutrition, nutrition education and meal rounding will be helpful for them. Especially the meals that the diabetes and low sodium diet patients are provided at the hospitals can be a model diet that the patients can follow when they are discharged and stay at home. Thus the patients should be educated on what they have eaten at the hospitals and why they have eaten them.;본 연구는 병원급식의 품질 속성에 대한 환자들의 인식 정도를 파악하고 병원 급식의 실제 섭취 수준을 조사하여 급식의 품질 수준 향상을 위한 방안을 마련하기 위해 수행되었다. 서울과 천안지역 종합병원 3곳에 입원한 516명의 환자들을 대상으로 설문조사를 실시하여 병원 급식의 섭취 실태와 급식 품질 속성에 대한 환자의 인식을 조사하였고, 같은 병원에서 3일에 걸쳐 집합선택계측법을 이용하여 잔반량을 측정함으로써 제공량 대비 실제 섭취 수준을 파악하였다. 자료의 분석은 SPSS Win (ver 12.0) 프로그램을 사용하여 실시하였다. 설문조사 결과 일반식(상식) 섭취 환자와 치료식(당뇨식, 저염식) 섭취 환자는 평균적으로 병원에서 제공된 식사양의 60-70% (밥류, 국류, 반찬류)를 섭취한다고 응답하였다. 응답한 상식 섭취 환자의 17%와 치료식 섭취 환자의 10%는 병원 식사를 남기지 않고 다 섭취한다고 응답하였다. 병원 식사를 남긴다고 응답한 환자들은 “맛이 없어서”와 “아프거나 기운이 없어서”를 주된 이유로 답하였고, 상식과 치료식(당뇨식, 저염식) 환자 간에 음식을 남기는 이유는 유의적인 차이가 없었다. 환자들은 급식서비스의 7가지 속성(온도, 맛, 위생, 메뉴의 다양화, 정확한 배식 시간, 배식서비스, 영양정보의 제공) 중 메뉴의 다양성, 일정한 맛의 유지, 식기와 음식의 위생 상태에 대해 부정적인 인식을 보였다. 특히 치료식 섭취 환자들은 상식 섭취 환자들에 비해 음식 온도의 적절성, 일정한 맛의 유지, 식사 및 영양정보 제공에 유의적으로 부정적인 인식을 보였다. 실제 계측을 통한 환자들의 병원 식사 섭취량 조사에서 상식(밥)의 제공량에 대한 섭취 비율은 3개 병원 평균 78.5%로 나타났고 당뇨식(밥)은 81.6%로 상식의 섭취량 보다 높은 섭취상태를 보여주었다. 저염식(밥)의 섭취율은 77.1%로 상식과 당뇨식에 비해 낮은 결과를 보였다. 이것은 저염식이 일반식에 비해 약 1/3가량의 염분이 함유되는 식사이기 때문이며, 본 연구를 진행하기 전 섭취율이 상식과 당뇨식 비해 낮을 것으로 예상했던 가정과 비슷한 결과였다. 따라서 염분제한식사에 대해서 환자들의 섭취율의 증가 될 수 있도록 많은 노력이 필요할 것으로 생각된다. 영양사에 의해 과학적이고 합리적으로 계획되고 제공된 환자식을 환자들이 다 섭취하지 못하는 것은 환자의 건강 유지와 치료에 부정적인 영향을 미치게 된다. 따라서 본 연구의 결과에서 나타난 환자들의 품질 속성에 대한 인식과 식사를 다 섭취하지 않는 이유들을 참고하여 병원급식을 담당하는 영양사 및 그 외 업무원들은 개선이 필요한 품질 속성에 대해서 집중적인 관리를 하여야 할 것이다. 환자의 컨디션과 기호도를 고려한 식단의 구성, 조리와 함께 적온 급식, 배선원의 서비스의 향상, 식사와 영양에 대한 정보 제공 등을 통해 환자들의 섭취를 증진시킬 수 있도록 제반 환경을 마련되어야 할 것이다.
