수면제의 임상적용 실태조사

Abstract

본 연구는 새로운 수면제 신약의 도입 후 실제 임상에서 약물이 적용되는 시간 경과를 파악하여 수면제 사용 실태를 연구하고자, 인천지역에 소재하는 가톨릭 대학교 성모자애병원에서 2002년 1월부터 2004년 12월까지의 트리아졸람과 개선된 수면제인 졸피뎀의 처방 건수와 약물 사용량의 자료를 근거로 통계 분석하여 다음과 같은 결과를 얻었다. 각 년도별 신경정신과와 내과의 수면제의 처방율과 사용량을 비교한 결과, 1. 졸피뎀의 처방건수는 트리아졸람의 처방건수에 비해 신경정신과와 내과 모두 통계학적으로 유의하게 증가하였다. 2. 트리아졸람의 처방율은 약간 감소하였고, 졸피뎀의 처방율은 신경정신과와 내과 모두 통계학적으로 유의하게 증가되었다. 그러나, 졸피뎀의 처방율은 내과보다 신경정신과에서 통계학적으로 유의하게 더 증가하였다. 3. 트리아졸람의 총사용량은 변화가 없거나 약간 감소하였으나, 졸피뎀의 사용량은 통계학적으로 유의하게 증가하였다. 그러나, 졸피뎀의 사용량은 내과보다 신경정신과에서 더 유의하게 증가하였다. 처방율과 총사용량을 비교해 볼 때, 내과에 비해 신경정신과에서 트리아졸람보다 개선된 수면제로 알려진 졸피뎀이 더 빠르게 임상에 적용되고 있었다는 것을 알 수 있었다. 실제 임상에서 효능과 안전성을 고려하고 신경정신과외 타과에서도 수면제의 사용이 적지 않은 현실에서 더 신속한 정보와 관심이 있어야 할 것이다.;This study was designed for exploring how many time it would be taken for new hypnotics to be applied for the patients after their being introduced, with using the number of prescriptions and the amount used in a university hospital, Incheon, Korea, from Jan, 2002 to Dec. 2004. The results were as follows; When the prescription rate and a total amount of the sleeping pills were compared between departments of neuropsychiatry and internal medicine, 1. The number of prescriptions of zolpidem compared to triazolam was significantly increased from 2002 to 2004 across both departments. 2. The prescription rate of triazolam was unchanged or slightly decreased, and that of zolpidem was significantly increased from 2002 to 2004 across both departments. But, the prescription rate fo zolpidem was significantly further increased in neuropsychiatry. 3. The total amount of triazolam was unchanged or slightly decreased, and that of zolpidem was significantly increased from 2002 to 2004 across both departments. But, The total amount of zolpidem was significantly further increased in neuropsychiatry. When it comes to the prescription rate and total amount of zolpidem, known as the better hypnotics than triazolam, the faster changing trend was shown in neuropsychiatry. comparing to internal medicine. Considering efficacy and safety for clinical purposes, it should be suggested that the information on hypnotic drugs be given faster and further for non-neuropsychiatry clinicians.
