고혈압이 동반된 제2형 당뇨병 환자에서 candesartan이 환자의 인슐린 저항성 개선과 혈중 MMP-2, MMP-9, TIMP-1 및 TIMP-2 농도에 미치는 영향

Abstract

연구목적: 고혈압 치료제인 안지오텐신 II type 1 수용체 차단제 중 candesartan이 단순한 혈압 강하의 효과를 넘어 인슐린 저항성을 개선시키고, 알부민 배설율을 감소시킨다는 보고가 있어 이 약물의 적용 대상이 넓어지고 있다. 본 연구에서는 제 2형 당뇨병 환자를 대상으로 당뇨병성 신증 환자에게 candesartan을 투약함으로써 환자의 인슐린 저항성 개선과 혈중 MMP-2, MMP-9, TIMP-1 및 TIMP-2 농도에 미치는 영향을 알아보았다. 대상 및 방법: 연세대학교 영동세브란스병원에 내원하거나 입원한 환자 중 당뇨병 환자로서 알부민뇨가 30㎎ 이상인 환자 29명, 당뇨병 환자로서 알부민뇨가 30㎎ 이하인 환자 33명, 환자군과 성별과 연령을 일치시킨 건강한 사람 49명을 조사하였다. 조절되지 않은 고혈압(uncontrolled hypertension) 환자, 만성 신부전증 환자, 악성종양 환자, 간질환자는 제외하였다. 선정 기준에 합당한 환자 중 111명을 대상으로 12주 동안 candesartan 8-16mg/day를 투여한 전향적 연구를 시행하였다. 연구의 시작 시점을 0주로 정하고 체중, 신장, 수축기 및 이완기 혈압, 생화학적 조사, 당뇨병의 유병기간, 24시간 소변검사, 혈청의 MMP-2, MMP-9, TIMP-1, TIMP-2 및 인슐린 저항성을 측정하였다. 결과: 당뇨병 환자에서 신증 유무에 따라 분류한 양 군간의 연구 시점에서의 임상적, 생화학적 특징은 알부민 배설율 외에는 차이가 없었다. 한편, 혈중 MMP-2, MMP-9, TIMP-1, TIMP-2 농도는 정상대조군을 기준으로 당뇨병 환자들에서 유의하게 감소되어 있었다. 치료 후, 당뇨병군보다 당뇨병성 신증군에서는 알부민 배설율의 유의성 있는 감소를 보였다. 치료 후 혈중 MMP-2, MMP-9, TIMP-1, TIMP-2 농도는 두 환자군에서 유의하게 증가하였다. 그리고, 알부민 배설율의 감소와 혈중 MMP-2, MMP-9, TIMP-1, TIMP-2 농도간의 유의한 상관관계를 보였다. 인슐린 감수성을 측정하는 수치인 ISI는 정상 대조군에서 보다 당뇨병군과 당뇨병성 신증군에서 감소되어 있었고 인슐린 저항성을 측정하는 수치인 HOMA-IR은 정상 대조군에서 보다 당뇨병군과 당뇨병성 신증군에서 증가되어 있었고 당뇨병군과 당뇨병성 신증군 사이에는 차이가 없었다. 결론: 연령, 성별, 당뇨병의 유병율, 당화혈색소, 혈압과 함께 혈중 MMP-2, MMP-9, TIMP-1과 TIMP-2 농도가 유의하게 분석되어, 혈중 MMP-2, MMP-9, TIMP-1과 TIMP-2 농도가 알부민 배설율과 연관성이 있다고 사료되며, candesartan의 투여는 당뇨병 환자에서 인슐린 저항성을 비롯한 이러한 인자들의 유익한 변화를 통해 혈압강하 효과뿐만 아니라 당뇨병성 신증의 치료제로서의 유용성을 시사한다.;Background: Diabetic nephropathy is the leading cause of end-stage renal disease. Candesartan, an angiotensin II receptor antagonist has been reported to have an antiproteinuric effect and increase insulin sensitivity with diabetic nephropathy. Objectives: The aim of this study was to test whether candesartan affects proteinuria, insulin sensitivity, and serum MMP-2 , MMP-9, TIMP-1, and TIMP-2. Methods: Total of 111 patients were enrolled in this study: 29 patients who have type 2 diabetes with an albumin excretion rate of over 30 mg/day, 33 patients who have type 2 diabetes with an albumin excretion rate of 30 mg/day or less and 49 control patients. 62 patients with type 2 diabetes took 8-16 mg/day of candesartan for 12 weeks and the following parameters were determined: weight, height, DBP, SBP, duration of diabetes, age, biochemical parameters, MMP-2, MMP-9, TIMP-1, and TIMP-2. Results: There was no significant difference in BMI, DBP, SBP, duration of diabetes, age, biochemical parameters except for albumin excretion rate between diabetic patients with and without nephropathy. The serum concentrations of MMP-2, MMP-9, TIMP-1 and TIMP-2 increased after the treatment in both diabetic patients. Insulin sensitivity also increased after candesartan treatment. Conclusions: Candesartan administration for 12 weeks was effective in decreasing hypertension, increasing insulin sensitivity and serum concentrations of MMP-2, MMP-9, TIMP-1 and TIMP-2 for both patients with diabetes and diabetic nephropathy. Proteinuria decreased for patients with diabetic nephropathy.
